Committee members filling exam committee positions must have the following:

· Have five years, full-time experience in aquatic facility operation or related area

· Successfully passed the AFO Exam and maintain a current AFO certification with NRPA OR be a current AFO instructor that has taught within the last 2 years

· Must have maintained a AFO certification for a minimum of 5 years (one certification cycle)

Please answer the following questions. If necessary, attach additional sheets.

2. Please list chronologically (starting with the most recent) experience with aquatic facility operation, involvement with setting national, state, or local aquatic standards, experience with national aquatics certification committees, or related experience.

3. Please list any related certifications (CPRP, CPRE, national aquatics certifications, etc.).

4. Please identify specific technical areas you consider as your strengths and weaknesses.

5. Please state why you wish to become a member of the AFO Exam Committee (200 words or less).
