Mt. Baker Bulldog Day

Students will be able to pick up their schedule, have their picture taken for the yearbook, pay elementary fines/fees and purchase their yearbook on Mt. Baker’s Bulldog Day, Tuesday, August29th. You will receive a newsletter in early August with the specifics. It is very importantthat all sixth grade students meet the immunization requirementsPRIOR to this day. We are not able to give schedules to those students who have not met the requirements. If your student gets their shotsduring the summer, please bringor FAXverification to

Attendance

A habit of good attendance is critical to success in middle school and high school. Be sure to establish the habit early in the school year. Every day is an important day. Students should only miss school if they are ill. Please schedule dentist and doctor appointments outside the school day. When your student is absent, please call the attendance office no later than

8:30 a.m. at 253-804-4557.

You will receive a newsletter in August with your specific time to pick up class schedules.
IMMUNIZATIONS: Students attending sixth grade will be required to show proof of immunizations:
  • Tetanus, Diphtheria, and Pertussis: 5 doses (4 doses only if 4th dose given on or after 4th birthday) plus 1 doseTdapis required ifstudent is age 11 or older.
  • Varicella (chickenpox): 2 doses or healthcare provider verification of the disease
  • IPV,OPV (Polio)4 doses (3 doses only IF 3rd dose given on or after 4th birthday)
  • Measles, Mumps, Rubella: 2 doses
  • Hepatitis B: 3 doses, dose 3 must be given on or after 24 weeks of age
If your child has not been immunized for any of the above vaccines, please contact your health care provider to make an appointment for your child to receive these vaccines.
We are unable to give schedules to those students who have not met these immunization requirements.
IMPORTANT: On or after August 14th, bring new shotrecords to Mt. Baker or FAX to 253-931-0661. This will avoid waiting in a long line on Bulldog Day.

Buy School Supplies early. (See list below)
Check to make sure your student has clothes to wear for PE class – shorts, sweats, tee-shirt/sweatshirt, socks and tennis shoes.
If your student is interested in participating in a sport during the school year:
  • Access athletic forms from the Mt. Baker Homepage (go to Activities/Click Athletics/Click Athletic Participation Forms).
  • Make an appointment for a physical AFTER July 1 but before school starts. Take the AthleticParticipation Form to the appointment and ask your physician to complete the Physician’s Report portion of the Athletic Participation Form.
  • Complete and sign all four (4) forms and return to Mt. Baker on Bulldog Day, August 29, 2017.

Purchase an ASB card: REQUIRED for athletics, band, orchestra, choir, drama and other clubs. (i.e., Robotics, Mathletes, Builders Club,Art Club, etc.)
Cost is$15.00. This can be done onlineafter August 21stor on Bulldog Day.
Purchase Yearbook: OPTIONAL- Distribution in June with student pictures, candid pictures of students and a place for friends to autograph. Available for purchase on Bulldog Day.Cost is $35.
School Pictures: Student Pictures will be taken on Bulldog Day, August 29th. Purchase of pictures is optional; however every student will have their picture taken for anID/ASB card and Yearbook. Picture packets will be mailed home with the August Newsletter.
