Describe Areas of Concern Based on Eligibility Determination: Marcus has a diagnosis of anxiety disorder and dyslexia from Iowa City Hospital and Clinics. He tends to become frustrated easily and shuts down. He may cry when very frustrated. Marcus has problems with word recall and has a very slow processing speed. His vocabulary skills are poor; however, his comprehension is good, especially when listening.

Marcus has difficulty with word recall and processes information very slowly.
Medications have been changed recently. Side effects and dosage will need to be monitored. / On tests and worksheets, Marcus will be given word banks to assist with word recall.
Marcus will be given extra time to complete daily work or will have number of items to complete reduced in half. He will have extra time to complete tests, including district and state assessments.
When asked a question in class discussions, he will be given wait time. If possible, the teacher will cue him about questions that he will be asked so that he can prepare to answer. He will be given visual or verbal cues to assist answering questions. The use of mnemonics or associations will be utilized in instruction.
No timed tests will be administered. In math, he can use a fact chart or calculator for basic fact recall.
Digital copies of social studies and science books and materials will be utilized.
A release of information has been signed to allow the school nurse to communicate with the doctor prescribing medication. Summaries of weekly work and behavior will be compiled by the school nurse and shared with the parent and doctor. The school nurse will maintain communication with all teachers about dosage and side effects. / Teachers
