**NOTE~ This form is due to the parish 4-H office by April 15th to qualify for service awards.

***All hours will be certified by the 4-H agents; don’t count hours helping family or any project you are paid to do.

The service must truly be a SERVICE and not a part of your daily chores or class responsibilities. 4-H’ers (8th grade and above) are expected to complete true service, not just “helping” activities. Donation of items to a collection drive is only worth 15 minutes of your time. You must donate to 4 different collection drives (no money drives allowed) to qualify for 1 hour of service time.
