4th Grade Graduation T-shirts

The school year is moving along quickly. It is a tradition for all of the 4th grade students to wear matching t-shirts with their names and graduation year proudly displayed on their backs to Move Up Day and other year end school functions, including the Pirate Game. No money is necessary to order a t- shirt. The 4th grade party money, collected at the beginning of the year, is used to purchase a shirt for each student. We would appreciate your help in choosing a size for your child. It is best to order a bigger size. Your son or daughter might wear this shirt for years to come.

Please return the order form below to Streams by February 20th. If we do not receive your order form by that day, we will order your student a size Child Large (14-16). T-Shirts will be distributed before Spring Break.

Also, please see the attached list of 4th grade students and check the spelling of your child’s name. If you would prefer using a nickname for the t-shirt or if there are any spelling mistakes, please indicate it on the form below. Please write legibly so there is no confusion.
