This form is the official notification to the HR Role Evaluation Team that a Police Staff role has been identified that requires consideration in relation to the evaluation of the grade for the role.

It is important to note that the date on this form will establish the date this process commences and will be used for administering any back pay.

It is important that the line manager:

  • contacts the IT Account Administration Mailbox in order that IT permissions are in place
  • seeks guidance for the level of vetting required for this role from the Internal
    Vetting Mailbox.
