Rationale: Present meeting and review previous conclusions.

Expected output: Establish scope and structure of meeting

Work Package Presentations

Rationale:To update on the work done by all WPs and discuss potential improvements for the future

Expected output:AB feedback and proposed improvement measures

Designation of AB rapporteur

Rationale: One member of the AB is meant to be elected rapporteur in order to: a) moderate the discussions of the AB, and b)present the AB conclusions to the GB

Discussion: Organising the flow of practices

Rationale:To discuss how to operationalize the exchange of good practices as a key element of JA-CHRODIS and how to achieve a strategic impact.

  • How to promote the use of the PKE amongst European health professionals. Exchanging good practices as the starting point to:

- Compare existing practices with evidence based recommendations

- Learn from each other

- Extend good practices and improve chronic conditions plans, programmes and interventions

- Issue recommendations on practices and training when appropriate

  • Transferability of practices: how to plan the transfer to improve the probabilities of success
  • Collaboration with other initiatives: how to make JA-CHRODIS the reference initiative on chronic conditions at European level

Executive Board members will attend this discussion to have the input of the AB.

Expected output:AB guiding ideas on the organization of a flow of good practices with an impact at European level

Workshop on PKE current developed functionalities (optional)

Rationale: PKE in current development. AB member are invited to test and provide feedback on PKE

Output: Feedback from AB on PKE
