Please complete the Excel form (digital format- no handwritten forms, please) with the information requested for each student. Provide the names of each paid student as listed on their school name badge. Please note that payments for all students from each school will need to be made with ONE check or Money order. No individual student registrations are to be made. Each school is to send the Excel listing of all students who are cleared (paid) to attend the convention. If their names are not on the list, they will not be admitted. At the convention, the students must wear their picture school ID badge to identify the student and their program. The student will sign in at the registration desk for their school, which should be manned by their school advisor/representative. The students’ school name badge will serve as their name badge for the convention. They must wear their name badge and their name must be on the registration list submitted by the school in order to attend the convention.
Instructions: MOSA Advisors, or designated school officials, will collect registration fees for students attending the MOADN convention. Sendthis form and a printed copy ofthe Excel list of student names, along with ONE CHECK OR MONEY ORDER from the school, to the address listed below. Individual forms for students are not required, as they will be registered as a group from each school. In addition, please e-mail the excel registration form to Donna Scoggin at .

Requests for refunds must be made in writing and postmarked on or before March 5, 2018 to the above address. Requests for refunds postmarked by the specified deadline will be honored, less a $10.00 processing fee. Refunds cannot be made for requests postmarked after March 5, 2018.
