2018 Candidate Questionnaire – Judicial Candidates

Thank you for your interest in being a candidate endorsed by the Chicago Federation of Labor.

The Chicago Federation of Labor represents more than 300 local unions who in turn represent more than 500,000 working men and women in Chicago and Cook County. This election cycle, the CFL will determine who best supports working families’ issues and merits Labor’s endorsement.

Your responses to these questions will help us make endorsement decisions and may be used to communicate the difference between candidates to union members. Please be candid, complete, and forthright in your responses. Questionnaires should be e-mailed to .

Completed questionnaires must be received by 4:30 p.m. on Thursday, December 14, 2017.

You may type your answers onto this questionnaire and use additional paper, if necessary, to fully respond to each question. Answers MUSTbe limited to one page.

The endorsement procedure is a two-step process. Once a completed questionnaire is submitted, candidates seeking an endorsement must interview with the CFL screening committee. Those screenings will take place during business hours on Monday, Dec. 18 Tuesday, Dec. 19 and Wednesday, Dec. 20.

CURRENT PROFESSIONAL EMPLOYMENT

  1. Who is your current employer? (Please include your employer’s name, address, and telephone number, the dates of your employment, and the names of persons familiar with the quality of your work for this employer.
  1. List all courts (including state bar admissions), and administrative bodies having special admission requirements, in which you are currently admitted to practice.
  1. Describe the general nature of your legal practice during the past ten years and identify any areas of concentration.

  1. Have you ever been the subject of any Attorneys Registration and Disciplinary complaints which resulted in a formal reprimand or other order?
  1. Has a client ever filed suit against you for malpractice? If so, what were the circumstances and outcome?
  1. Have you ever been arrested or charged by federal, state, or other law enforcement authorities for alleged violations of any federal, state, county, or municipal law, regulation or ordinance?
