2017 Pledge, Code of Code, Release and Waiver

Player Pledge/Code of Conduct: I, ______, understand that I will be expected to attend all practices and games. I will respect the coaches and officials participate under their direction, and practice good sportsmanship at all times. I understand the coaches have the discretion to start or substitute players at their will. I will abstain from the possession and use of any alcohol, drugs and/or tobacco products and will seek assistance in dealing with any chemical dependency issues. I will wear all of the necessary and required equipment for participating in lacrosse. The Parkway West Lacrosse Club (the “Club”) will not tolerate any acts of vandalism or actions that result in damage to property or any Parkway West facility or other teams’ facilities. Knowing that athletics are a privilege, I accept and pledge to abide by this Code of Conduct and other rules set forth by the coaches and the Club. I pledge to honor the game.

Parent/Guardian Pledge/Code of Conduct: I understand that I will be expected to be a good representative of the Club. During games, I will provide only positive and encouraging cheers. I will not argue with or in any way seek to antagonize coaches, officials, other teams’ members or other spectators. I will not coach from the bleachers. I will support the coaches and will contact the coaches or board members at the appropriate time and place with any questions or concerns. I pledge to honor the game.

Release/Waiver:
I fully understand that playing lacrosse poses certain inherent risks and hazards to my son, including but not limited to: risk of injury, including broken bones, sprains, contusions, abrasions, and injuries to the head, neck, back and extremities; bodily injuries that may result in permanent disability and/or death; equipment failure or malfunction (my player’s or others); exposure to elements and temperature extremes resulting in frost nip, frost bite, heat exhausation, heat stroke, sunburn, hypothermia, and/or dehydration; fatigue, exhaustion, chill and/or dissiness; diminished reaction times; and, impact or collision with other athletes, spectators, coaches, and officials. The Club, its team members, coaches, represtatives, and board members are hereby absolved from all responsibility for all consequences in the event of a medical emergency. The Club will do its best to reasonably provide for prompt medical care in the case of an emergency.
YES, I do hereby affirm and acknowledge that I have been fully informed of the inherent hazards and risks associated with my son, ______, participating with the Club. I agree for ourselves, our heirs, executors, and administrators, to release, hold harmless, and forever discharge the Club, its officers, directors, board members, coaches, volunteers, sponsors, and other associated or affliated representatives or agents, from and against any and all claims, actions, cause of actions, suits, judgements, and demands whatsoever arising directly or indirectly in connection with my son/player’s participation in and with the Club. By signing below, I acknowledge that I have fully read and understand this form and further understand the terms herein and contractual and not a mere recital.
