2017-2018 VITA/EITC VOLUNTEER AGREEMENT

As a volunteer for the Volunteer Income Tax Assistance (VITA)/Earned Income Tax Credit (EITC) Program, I understand that the County of Alameda is allowing me to attend training during work hours without having to use personal leave time. As a provision of my county paid tax preparer training, I agree to the following program requirements stipulated in this agreement. I understand that if I fail to meet all training attendance requirements that I will be required to use personal leave (vacation, floating holiday, or management paid leave, if applicable) for any time away from work and training. I further understand that if I fail to complete the required 30 (minimum) volunteer hours I will be required to use personal leave (vacation, floating holiday, or management leave, if applicable) to cover any county paid training provided. The required 30 volunteer hours (this does not include mandatory training hours) are completed on the volunteer’s own time. County leave balances cannot be used for volunteer hours.By initialing each statement below and signing this document I agree to adhere to all stated attendance and time reporting provisions, as well as volunteer requirement:

Session 1: January 2 – 5, 2018: Customer Service/ Tax Preparation Training

Session 2: January 8 – 11, 2018: Customer Service/Tax Preparation Training

Session 3: January 16 – 19, 2018: Customer Service/Tax Preparation Training

Session 4: January 22 – 25, 2018: Customer Service/Tax Preparation Training

OR

_____I am a RETURNING volunteer. I agree to attend Customer Service/Refresher trainingin order to participate as a Tax Preparer and certify to the Advanced Level. I agree to attend Customer ServiceRefresher AND Customer Service/Screener to participate as a Tax Preparer/Screener and certify to the Advanced Levelin the VITA/EITC program.*Screeners must have at least TWO year experience, and are requiredto prepare five (5) tax returns during tax season.

_____ I am aware that volunteer hours are unpaid.

OR

_____I have TWO OR MORE years of experience as a tax preparer. I agree to complete theLink and Learn Online Training and certify to the Advanced Level by COB January 12, 2018. I will provide my signed Volunteer Agreement showing certification level to the appropriate site coordinator.

AND

_____I am aware that there are no make-up sessions.

_____I have received prior permission from my supervisor to attend training.

_____I understand that if I fail to attend training and do not report to work I will be required to report time away from the office as personal leave. Note: This provision is waived for reported and excused illness.

_____I will complete a minimum of 30 volunteer hours during the period of January 22, 2018 – April 30, 2018.
