DV Lacrosse, Inc.

Desert Vista Lacrosse, Inc was established with the Arizona Corporate Commission and has been granted IRS tax exemption under 501(c)3. We are a public charity. The goals and purpose of the Club are to financially and administratively support the boy’s youth lacrosse teams that fall under the auspices of the Desert Vista Lacrosse Club. We are a Committee based organization that relies 100% on our volunteers. Booster Club Meetings, held 1x/month during the school year, provide a forum for updates, decisions and voting. Established By-laws govern the club’s activities and expenditures. All parents/guardians of players are Booster Club Members with voting rights (2 per family).

Presidents
(co-Pres) / Derick Rodriquez
Megan Egan / Presides over all Board/Booster meetings utilizing a pre-published agenda. Ensure that the Board is in compliance with the 501©(3) requirements, the By-Laws, AYLL Regulations and Desert Vista Thunder Board Guidelines.
Vice President (co-VPs) / Tc Macey
Alan Shank / Assist the President in all duties. Liaison between the committee chairs and the Board and will present committee reports when necessary.
Secretary / Laura Moore / Take minutes at each meeting and post them in a timely manner. Responsible for distributing pertinent information to the Club.
Treasurer / Lisa Rivera
Patty Royal / Maintain the Desert Vista Lacrosse accounts
and provide an updated Budget at each Booster Meeting. Files Annual Corporate Report and Tax Forms.
Thunder Board / Michelle Mace / Attends all Thunder Board meetings and reports back to the Club. Works with our school sponsor as well as the Desert Vista High School Administration.

DVHS Boys Lacrosse Activity Club

Our club is a DVHS Activities Club. As such, student officers are elected by members. Meetings are held outside of practice a few times per year at DVHS. Our DV Sponsor is Laura Bruner.

DV Lacrosse Committees

Committee Process

Established committees shall carry out specific assignments for which they were formed. It shall be the responsibilities of these committees to plan and communicate with the Board and Booster Club members.

·  When a committee is active, updates are required at the monthly Booster Club Meetings.

·  All decisions regarding facility usage, agreements, commitment of funds and/or student body must receive prior approval from the Booster Club during regular meetings or the President outside of regular meetings. The President will follow established by-laws to determine the appropriate approval process outside of Booster Club meetings.

·  Vendor contracts must be reviewed and approved by the President and one other Board Member.

·  Committees must establish and adhere to a budget approved by the Treasurer, prior to any expenditures or vendor commitments. The Treasurer will determine if a Budget review is necessary per the by-laws.

·  All monies collected shall be remitted to the Treasurer within ten business days of receipt.

·  All expenses to be paid with personal funds must be approved by the Treasurer in advance. Expenses in excess of $1000 must be paid with club funds only after the Treasurer reviews the invoice.

Committee Volunteers

Committee Chair positions and Committee Member positions are volunteer positions. Committee chairpersons may volunteer for consecutive one year terms. In the event that a chair position is vacant, the Board will appoint committee chairs. Tenure of office for a committee chairperson shall cease upon the completion of the season or activity for which they may be responsible.

The duties of a committee chairperson shall be to chair and coordinate the activities of that committee. When funds are involved (expense or collection of funds), the Chair must ensure the Committee Budget is approved by the Treasurer in advance. Any changes to the approved budget must be immediately presented to the Treasurer for approval.

Committee / Responsibilities
Banquet / This committee plans, coordinates and executes the year end Awards Banquet. Typically this committee has multiple sub-committee, established annually (ex: Venue, Gifts, Decorations, etc..) The committee chair ensures the budget is pre-approved and manages each subcommittee, providing regular updates to the Booster Club.
By-Laws / Committee to review by-laws and present any changes to the Booster Club for a vote.
Club Manager / There will be a designated Club Parent/ or Board Member who will coordinate efforts across teams (partnering with team parents) to ensure consistency, where appropriate, and to build club cohesiveness.
Community Service
Patty Royal / This committee assists players to identify and plan community service opportunities for players.
Equipment Manager / The equipment manager will ensure equipment is purchased per the approved budget and available to coaches throughout the season (ex: balls, nets, paint, etc..) The equipment manager will coordinate with coaches to proactively identify additional equipment needs and bring requests to the Board.
Field Coordinator
Lisa McLaughlin / This chair coordinates with coaches and the City of Phoenix Ahwatukee Lacrosse Field Coordinator to schedule all City of Phoenix fields. They will review team schedules with field schedules to ensure accuracy. If fields other than City of Phoenix are needed, the chair will coordinate securing of fields, applications, insurance documentation and payments with a Board Member.
Field Maintenance / This committee ensures fields are lined, sets up nets for games and escalates any field issues to the Field Coordinator. Ideally, the committee is comprised of at least one volunteer from each team.
Game Film & Photos / Ideally, each team has volunteer(s) to film games and take photos to be posted on designated website.
Fundraising / This committee plans, coordinates and executes our annual major club fundraiser. Typically this committee has multiple sub-committee, established annually (ex: Venue, Silent Auction, Gambling, Decorations, etc..) The committee chair ensures the budget is pre-approved and manages each subcommittee, providing regular updates to the Booster Club.
Lax4Life / This committee plans and executes our annual community fundraiser. Typically this committee has multiple sub-committee, established annually (ex: Venue, Concessions, Spirit wear, Registration, Team Coordination, Clean Up, DJ, Beneficiaries, etc..) The chair ensures the budget is pre-approved and manages each subcommittee, providing regular updates to the Booster Club.
Legacy Membership
Lisa McLaughlin / Establishes an alumni contact list to communicate significant events.
Other Fundraising / All fundraising activities require a chair to coordinate planning and execution. These committees plan, coordinate and perform, on behalf of the Booster Club, tasks to raise additional funding. The chair ensures the budget is pre-approved and manages each subcommittee, providing regular updates to the Booster Club. Examples of past committees are: Decals, Restaurant Night, eScrip (Fry’s and Amazon Smile), and 50-50 Raffle. (monies that can be tracked goes toward $250.00 Fundraising in Spring)
Registration Chair
Patty Royal / Partnering closely with the Treasurer , the Registration Chair will create the online registration. All content must be pre-approved by the President and Treasurer. The chair will ensure all players attending practice are registered in a timely manner. The chair will assist the Treasurer in collecting funds.
Spirit Wear / This committee plans, coordinates and executes spirit wear sales. Spirit wear is offered in the Fall & Spring as bulk team orders. The chair provides updates and seeks selection feedback in the Booster Club Meeting. The vendor selection, product selections, and pricing must be approved by the Board and the budget must be pre-approved by the Treasurer before the store is opened.
Team Apparel
DV Board / This chair/committee manages bulk team apparel purchases (ex: DV jackets, bags, pinnies, practice wear, etc..). Designs & Quantity should be presented to the Booster Club where possible and must be pre-approved by the Board and Head Coach. Expenses, Pricing and Order/Payment Method must be pre-approved by the Treasurer.
Team Parents / Each team will have a designated Team Parent who will coordinate volunteers and team activities to support that team. The Team Parent will work with the Coaches to communicate proactively and effectively with the players & parents.
Webmaster / Partners with Board and Coaches to maintain website communications, team rosters, statistics and more. Assists Committees as needed with website functionality as needed. (ie.. online stores, etc..).
