Team Data Form for Official Souvenir Program & Press Booklet

Schools whose softball teams advance beyond the regional quarterfinals must complete and return this three-page team data form, along with a team photo, to Royal Publishing via e-mail () not later than 5 p.m. EST Monday, April 27, 2015.

MANDATORY ELECTRONIC TRANSMISSION: This team data form must be completed and transmitted electronically via e-mail to “”. A digital team photo also must be transmitted as an e-mail attachment to the same e-mail address (type the words “NO SPAM” and your school name in the subject line to avoid having your email get caught up in the spam filter). The team photo should be saved in JPG format with a resolution of 150-300 dpi (300 is preferred) or in TIFF format with a resolution of 300 dpi. A school that fails to meet the appropriate deadline shall be assessed a $50 late fee. If the school advances to the State Championships and the materials are not received by the FHSAA Office’s publication deadline and, therefore, do not appear in the souvenir program, the school shall be assessed additional fees up to $50. SHOULD A SCHOOL NOT GET CONFIRMATION OF RECEIPT OF MATERIALS, OR CALL TO CONFIRM RECEIPT OF MATERIALS, THE FHSAA IS NOT RESPONSIBLE FOR THE LOSS OR NON-RECEIPT OF THESE MATERIALS.

MANDATORY SUBMISSION OF UP-TO-DATE STATISTICS: The 32 schools advancing to the Florida High School Softball State Championships must report up-to-date cumulative statistics through the Regional Tournament Championship Game by completing and returning the “2015 Florida High School Softball State Championships Up-To-Date Team Statistics” form, available for download on the FHSAA web site, to the FHSAA Office via e-mail () not later than 5 p.m. EST Monday, May 4, 2015. Each qualifying school that does not meet the deadline shall be assessed a $50 fine.

Instructions on Using This Microsoft Word Form

1.Save and name this form according to your school name and classification. For example, Miami High School would name its form "8A_miami.docx". Wesley Chapel High School would name its form "5A_wesley_chapel.docx".

2.Type in all of the information requested on the form. Type the information into the white fields. Avoid typing in the gray highlighted areas. Avoid causing one of the white fields to break or expand beyond the space allotted. Move from table to table in the form using your mouse and cursor. Move from field to field in each table using the "tab" key.

3.Double-check and triple-check all information, especially the team roster information. The information you provide will appear in the official souvenir program exactly as you provide it.

4.Save frequently as you complete the form to avoid losing your work.

5.Once you have completed the form, save it. Print a hard copy for your files.

6.To file the completed form with the FHSAA Office, you must: E-mail the Microsoft Word file as an attachment to "". In the subject field of the e-mail, type in the name of your school, the sport and word "form" – also include the words “NO SPAM” in the subject line to avoid having your email get caught up in the spam filter. For example, South Lake High School would type in "South Lake softball form NO SPAM" in the subject field.

If you receive an e-mail response confirming receipt, YOU DO NOT HAVE TO CALL, but please keep a copy of the response on file for your records. Only if you have not received a confirmation e-mail within 24 hours call 800-397-5834 and press 2 for the Editorial Department.

7.To file your team photograph with Royal Publishing, you must: E-mail your team photograph as a separate e-mail attachment to “”. The digital photo should be saved as a JPG file with a resolution of 150-300 dpi (300 is preferred) or a TIFF file and have a dpi of 300. Do not crop the photo. Royal Publishing will crop the photo to fit its layout specifications. In the subject field of the e-mail, type in the name of your school, the sport and the word "photo". For example, Dwyer High School would type "Dwyer softball photo" in the subject field.

If you receive an e-mail response confirming receipt, YOU DO NOT HAVE TO CALL, but please keep a copy of the response on file for your records. Only if you have not received a confirmation e-mail within 24 hours call 800-397-5834 and press 2 for the Editorial Department.

8.Just in case we haven’t emphasized this point enough, if you do not receive confirmation via e-mail call 800-397-5834 and press 2 for the Editorial Department to confirm receipt of your team data form and team photo. 4) Tournament Roster

Provide the jersey number, first and last name, position, height, class and bats/throws left- or right-handed for up to 25 players. For class use 6th, 7th, 8th, Fr., So., Jr., Sr. Also, provide the following final regular season statistics for each player: runs scored (R), home runs (HR), runs batted in (RBI) and batting average (Avg.). Do not include statistics from preseason classic tournaments, district tournaments or regional tournament games. Double check and triple check your spellings.

Provide the jersey number, first and last name, height, class (i.e. Fr., Jr., etc.) and throws left- or right-handed for each member of your pitching staff. Also, provide the following final regular season statistics for each pitcher: innings pitched (IP), hits allowed (H), walks (BB), strikeouts (K), wins, losses and earned run average (ERA). Do not include statistics from preseason classic tournaments, district tournaments or regional tournament games. Double check and triple check your spellings.

Use the space below to report any interesting facts or statistics about your head coach, individual players, team or school in general that might be of interest to news media covering the FHSAA Softball Finals. It's ok if the news and notes end on the next page.

List the scores of each game played during the regular season, including invitational tournaments, etc., as well as scores from District Tournament games through the championship game. Indicate games played in tournaments with an asterisk (*). DO NOT LIST GAMES FROM PRESEASON CLASSICS. Such preseason games must not be counted on your team's won-loss record.
