Your 2015-2016 Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification. The law says that before awarding Federal Student Aid, we must ask you to confirm the information you reported on the FAFSA. To verify that you provided correct information, the financial aid administrator at WTAMU will compare your FAFSA with the information on this worksheet along with copies of your (your spouse’s and/or parents’) 2014 Federal Tax Transcripts with W2s and with any other required documents. If there are differences, your FAFSA information may need to be corrected. You (and at least one parent if you are a dependent) must complete and sign this worksheet, attach any required documents, and submit the form and other required documents to the financial aid administrator at WTAMU. WTAMU may ask for additional information. If you have questions about verification, contact the financial aid office at WTAMU as soon as possible so that your financial aid will not be delayed.

  1. Complete this section if someone in the student’s household (including parents) received benefits from the Supplemental Nutrition Assistance Program or SNAP (formerly known as food stamps) any time during the 2013 or 2014 calendar years.

Either the student or student’s parent’s received SNAP benefits in 2013 or 2014.

*If we have reason to believe that the information regarding the receipt of SNAP benefits is inaccurate, we may require documentation from the agency that issued the SNAP benefits in 2012 or 2013

Either I (or my spouse, if married) or (one (or both) of the student’s parentspaid child support in 2014.
