IMPORTANT INSTRUCTIONS FOR COMPLETING APPLICATION

Attach a resume AND a brief narrative (1 page or less) stating why you should be considered for an HHS scholarship. Include in the narrative a description of your scholarly accomplishments (e.g., research presentations, publications, grants, awards), participation in professional activities, service to your department/university and/or community, and your career goals. Additional information may be provided at your discretion. Information within your narrative may be shared with your scholarship donor if you are chosen to receive a scholarship.

Some scholarships are based on academic merit alone while others are based on merit and/or financial need. Therefore, in order for final determination to be made regarding scholarships and your eligibility to receive a scholarship based on financial need,you must complete the Free Application for Federal Student Aid (FAFSA). If you wish to be considered for merit-based scholarships only, you need not complete the FAFSA. Final scholarship awards will be determined by departmental faculty representatives and will be announced in May. UNIVERSITY POLICY: A scholarship recipient must be enrolled as a full-time student (Undergraduate – 12 hours; Graduate - 9 hours) during the semester(s) the funds are awarded. Scholarship recipients must be declared majors in the School of HHS; changing your major may result in cancellation of your scholarship. A scholarship is granted for one academic year only, for the period of August through May. Scholarships in excess of $750 awarded will be split in half with one-half applied in fall 2015 and one-half applied in spring 2016. Additionally, during the fall semester that your scholarship is applied, you will be provided with your donor’s contact information. It is required that you follow up by writing a thank you note to him/her, and file a copy of this note in the HHS Dean’s Office.

Scholarship recipients will be notified in May2015. HHS scholarship awards will be applied to fall and spring semesters only. Funds generally are not available for summer school.
