2017 Olive Chapel Swim Team Registration

Please fill out the following requested information completely and bring to registration. The registration fee is $100.00 for first child, $95.00 for any additional children (a swim cap and team t-shirt are included).

There will be a minimum $25.00 non-refundable fee for each child who decides not to participate. The last day for partial refund is June 6th, 2017.

All parents are required to volunteer for 4 of the 6 swim meets. This year volunteer assignments will be done during registration. You will be able to choose what you would like to do and at which meets. It will be based on first come first serve the day of registration. Please refer to the volunteer job description on the website. Your child will not be registered on the team until your volunteer assignment has been made.

If this is your child’s first season with the Olive Chapel Otters, you must bring a copy of their birth certificate for verification of age.

If you are unable to come to registration, please mail registration form along with checks and copy of birth certificate, if necessary. You may indicate 4 choices for your volunteer assignments along with dates available. After April 30th all volunteer positions will be subject to assignment by the volunteer committee.

In order to participate on the team, ALL children must be able to swim the length of the pool without aid.

Note: New six and under swimmers will register for the swim team just like all other swimmers. These new swimmers will be swim tested on or before the first day of practice. If they are ready to join the team they will begin practicing with the returning 6 and under swim group. If the new swimmer needs a little skills reinforcement before joining the returning 6 and under practice group they can register for the Otter Pup Program for an additional $25. During the first few weeks of the season they will attend organized swim team prep sessions until they can pass the swim test outlined in the Beginner Swimmer Policy. Details of this policy can be seen at . There will be no refunds for swimmers who participate in the Otter Pup program but are unable or choose not to join the team.

- I will not interrupt the coaches during practice or meets.

- I will do my best to ensure my swimmer(s) arrives on time for practices and meets.

- If my swimmer(s) cannot attend a meet for any reason, I will notify the coaches.

- I will volunteer to work at 4 swim meets or meet equivalent positions.

- If I cannot fulfill my volunteer duties as scheduled, I will find a replacement and notify the volunteer coordinator of the change.

- I respect the coaches’ right to suspend my child’s participation in practice or meets as a disciplinary action.

- I have read the HandBook(located on the otters website) and agree to abide by the swim team policies.

Waiver

I, my heirs, executors, administrators and assigns, hereby release the Dogwood Ridge and Pearson Farms Homeowners Associations, Tarheel Swimming Association, pool managers and operators, volunteers and coaches from the above named participants’ injury or loss which might occur during participation and travel to and from planned events, including injury or loss arising from or contributed to by any physical impairment or defect that said participants may have, whether latent or patent, and I agree that sponsors and promoters are under no obligation to provide physical examination or other evidence of said participants’ fitness to participate in such activities, the same being my sole responsibility.
