Methodology used to collect trip data:

We use spreadsheets that are updated daily to collect passenger information and for fare collection tracking. We also keep hard copies (paper files) containing the same information. We have also created a statistics sheet, used by the three agencies to use as a quick reference. The above data is collected from these materials.

2.  Outcome Measures:

Required Outcomes – Primary Goals / Deliverable / Progress
Services provided that impact availability of transportation services for individuals with disabilities. / ·  Horizons and Senior Center are continuing to meet and coordinate to discuss and evaluate our services, progress, transportation issues and challenges. At the meetings we discuss how to resolve our issues so we can better serve the target population.
·  Horizons and Senior Center meet the demands of transportation requests from clients with a disability / ·  Horizons, SEA and Senior Center met once this month with Society for Equal Access and the Tuscarawas County Senior Center to evaluate our services and work out problems.
·  Senior Center has made third wheelchair van available on an as-needed basis
Additions or changes to environmental infrastructure (transp. Facilities, side walks, etc.) technology (radios, computer software, etc.) and additional accessible vehicles. / ·  Horizons - additional expense of seven (7) new cell phones for each new van, used to protect passenger confidentiality due to HIPPA laws. Existing radios improve communication, and phones allow dispatch/drivers to protect passenger information.
·  Senior Center - apply for Specialized ODOT grant for new modified vehicles / ·  Horizons - Radios have already expanded communication capabilities between drivers and dispatchers and have certainly allowed for a great deal of growth within Horizons and New Freedom transportation. Cell phones protect consumer confidentiality.
·  Senior Center - submitted 2011 Specialized grant for 4 new vehicles on Jan. 28, 2011
·  Senior Center - Received to date 3 modified vehicles from the 2010 specialized grant
Local Outcomes / Deliverable / Progress
·  Horizons and Senior Center - Providing additional transportation to those who may otherwise be unable to travel
·  Horizons and Senior Center - Increase efficiencies of coordinated transportation in the County & increase community awareness among non-users
·  Senior Center – Increase of 175 trips over a 12 month period (Jan – Dec 2011)
·  Senior Center - Generate $1000 in fund raising to support New Freedom transportation program / ·  Horizons and Senior Center - We are able to take people to their desired destinations
·  Horizons – Increased service hours to transport earlier in the morning to get more people to work earlier
·  Horizons and Senior Center - Increase number of Brokered Trips
·  Serve 300 new clients (combined)
·  Senior Center – 2,063 trips taken in 2010. Increase total trips for 2,238 in 2011
·  Senior Center - Currently planning fund raiser / ·  Horizons and Senior Center - There is less duplication of services and more of the target population is being served.
·  Brokered Trips through November is 676 (combined)
·  Exceeding goal, YTD new riders is 442 (combined)
·  Senior Center – YTD number of trips 1,894
·  Senior Center - Goal for local match money 2011 has been met – Senior Center conducted solicitation fund drive to support Mobility Manager grant. Raised over $1200 YTD.

3. Have there been any service changes during this reporting period? If so, please describe. (Remember that service not covered in the originally-approved scope required ODOT approval before the change is implemented).

None to report at this time

4. What marketing efforts have been implemented during this reporting period? Please include the number of audiences which have been targeted e.g. Chamber meetings, rotary meetings, etc. Please provide copies of any printed materials regarding your New Freedom project.

·  Horizons had QAR review with ODOT personnel to discuss progress, outcomes and ways to improve upon New Freedom / Tusctransit program. Horizons developed and implemented recommended changes and improvements to New Freedom Program based on QAR requirements and recommendations.

·  Horizons and Senior Center attended Coordination Meeting November 3, 2011

·  Horizons Executive Director attended Tuscobus meeting November 9, 2011 to discuss progress and development of Tusctransit program

·  Horizons, Senior Center, SEA and Job and Family Services participated in coordinated Defensive Driving Training on November 16, 2011 (instructed by Carole Baker, AAOA)

·  Senior Center & Horizons – No extra marketing this month. Continue to announce New Freedom in vehicles and newsletters.
