US Youth Soccer strives to conduct events of the highest caliber and is always looking for ways to improve our events. We appreciate you taking the time to assist us in that goal.

Please use the following guidelines when submitting your report to yourRegional Cup Chair. Please note that failure to turn in your report could result in the loss of a portion or the entire amount of your team’s subsidy. National League Teams please submit your report to the National Office via (). If you answer yes, please elaborate. If your answer is No, please explain why.
