PART I. ACADEMIC EXCELLENCE (25 Points)
Identification, surveys or examples indicating the Inn's academic performance. These may include grade point averages, number of students on law review or in scholastic honorary organizations (such as Order of the Coif); legal articles published by members; individual academic performances; scholastic award winners; requirements for admission to the Inns, etc. Include a description of the system for academic recognition available at particular law school (letter from law school or Province President).

PART II. INN ACTIVITIES (25 Points)
Describe all projects conducted by the Inn for the benefit of the law school and the community.
PART III. INDIVIDUAL MEMBER ACHIEVEMENTS (25 Points)
Identify members on Law Review, Student Bar, Moot Court and other organizations which tend to indicate achievement by members of the Inn within the law school as well as achievement by members of the Inn within the community.

PART IV. SINGLE PROJECT AWARD (10 Points)
Select one project, which you believe to be the most outstanding effort conducted by the Inn and describe the matter in detail. The Court of Appeals may, in its discretion, give a special award for single projects if it deems such award appropriate.
PART V. IMPROVEMENT (15 Points)
Indicate the extent to which activities in the current year and the general health of the Inn constitute an improvement over performance in the previous year. Alternatively, if it is your judgment that the Inn has maintained a consistently high standard, indicate how the performance of your Phi Delta Phi Inn compares with other fraternities or organizations on the campus.
