¾ FORM MUST BE TYPED!!! Email form to . DO NOT EMAIL PRIOR TO 5PM ON FEBRUARY 5.
¾ Form must be received by Georgia HOSA no later than 5pm on February 10. Lodging assignments will be made based on the chapter’s lottery assignment. Forms not received by the deadline will be re-assigned to the last lottery number available at the time the form is received.
¾ You must make lodging reservations through the Georgia HOSA Hotel Reservations Desk to stay at the hotels below. Members must be registered for the conference before hotel assignments will be made. No phone reservations accepted. If you do not reserve rooms through this method, you will not be allowed to attend the State Leadership Conference. Any rooms reserved by chapters outside of the lottery will not be honored and the booked rooms will be dropped from the hotel. fi
¾ You must mail deposit to hotel upon reservation confirmation (check made payable to hotel). Payment details must be confirmed with your assigned hotel. Georgia HOSA will not handle any hotel payment issues.
¾ If you qualify for tax exemption, you must submit a completed copy of your tax exempt form to the hotel directly.
¾ In the Name of Guests section, indicate if each room is an adult or student room, male or female room and check the checkbox if you would share that individual room with another chapter who is willing to share their room. Please indicate the minimum number of room (for example, if we were able to combine your students into more than 4 per room, how many rooms would you be comfortable with?)
¾ If you are traveling with another school and are required to stay at the same hotel, you must submit ONE COMBINED FORM for multiple schools, with all lodging needs. All registered chapters must be represented on a form to participate in the conference. Any schools submitting together will take the lowest lottery assignment of the schools within the request.
¾ Each room must have names listed to be accepted. Rooms will not be assigned unless there are names on the room. Remember that if there is one name on a student room, they could be condensed with another room if larger rooms are available.
¾ Some hotel rooms do have a sleeper sofa, which is considered in the number of people the room can accommodate. For example, a room with two queen beds and a sleeper sofa may be assigned up to 6 occupants.
¾ I have reviewed the dress code and code of conduct with everyone registered to attend the SLC with our chapter.
¾ Please make sure to follow all directions on this form very closely. If the submitted form is not completed correctly and in it’s entirety and we cannot reach you after 5pm on February 10 by the number listed in the contact information, your chapter will be skipped and moved down the list until you have been contacted.

Once your hotel assignment has been made, you will receive an email listing your hotel assignment. If you are paying the deposit by check, you must then send a check directly to the hotel made payable to the applicable hotel. Instructions will be sent in your assignment email. Forms not received by 5pm on February 10th will be moved to the bottom of the list. If a form is not received by your chapter, you will not be eligible to attend the SLC. It is your responsibility to contact the hotel to establish a payment method. If you don’t contact the hotel with your method of payment, rooms may be dropped.
