Award Nomination Rules:
1. Nominate only one person per form in all of the award categories below that may apply. The more categories nominated in, the better chance of being awarded.
2. Be as accurate as possible with spelling of name, certification levels, affiliation, etc.
3. Please type or print legibly.
4. Only nominations with a documented reason why they deserve this award will be considered.
5. As a word document this can be filled out on the computer and emailed to the SPEMS office.
6. A survey monkey link to this form is on the SPEMS website for online submission.
7. Nominations must be received at the SPEMS office no later than COB Friday, January 19, 2018.

SPEMS Award Categories

EMS EDUCATOR Award honors an EMS Instructor or Course Coordinator who has advanced EMS education in the South Plains Region.

PHYSICIAN Award honors the physician that is dedicated to patient care, assumes a leadership role, and is an EMS advocate.

NURSE Award honors a nurse who has excelled in the areas of patient care and leadership, and is an advocate for EMS.

EMS ADMINISTRATOR Award honors an administrator, researcher, or manager on the local, city, county, or regional level who has made a positive contribution to EMS.

FRIEND OF EMS Award honors a private citizen for a heroic lifesaving act or unique advocacy of EMS.

PUBLIC/PRIVATE PROVIDER Award honors an organization that took a leadership role in EMS by achievement in areas of patient care, public access, medical control, disaster preparedness, public education, and training.

VOLUNTEER PROVIDER Award honors an organization staffed primarily by volunteers that assumed a leadership role in EMS by achievement in areas of patient care, public access, medical control, disaster preparedness, public education, and training.

FIRST RESPONDER ORGANIZATION Award honors a first responder organization that assumed a leadership role in EMS by achievement in areas of patient care, public access, medical control, disaster preparedness, public education, and training.

TELECOMMUNICATOR Award honors a person or team who handles a call or system event with professionalism and efficiency that allowed the first responders on the scene to give the patients the best patient care possible. An individual or a team is eligible for the award.

OUTSTANDING EMS PERSON OF THE YEAR Award honors an EMS certified person who has demonstrated uncommon leadership and courage in providing emergency medical service to the citizens of the South Plains Region.

EMERGENCY CARE ATTENDANT OF THE YEAR Award honors the ECA who has excelled in the areas of patient care, service to their EMS organization, service to the community, service to EMS in the South Plains Region.

EMERGENCY MEDICAL TECHNICIAN OF THE YEAR Award honors the EMT-B who has excelled in the areas of patient care, service to their EMS organization, service to the community, service to EMS in the South Plains Region.

EMERGENCY MEDICAL TECHNICIAN INTERMEDIATE OF THE YEAR Award honors the EMT-I who has excelled in the areas of patient care, service to their EMS organization, service to the community, service to EMS in the South Plains Region.

EMERGENCY MEDICAL TECHNICIAN PARAMEDIC OF THE YEAR Award honors the EMT-P who has excelled in the areas of patient care, service to their EMS organization, service to the community, service to EMS in the South Plains Region.

MARK BECK HUMANITARIAN Award honors an individual who exhibits acts of kindness, generosity and /or heroism-an individual who is committed to EMS and his/her community.

SCOTT MURRY LIFETIME ACHIEVMENT Award honors an individual who has spent a lifetime of dedication to patient care. This award may not be presented every year as it is the intent for this award to be given to an outstanding individual who has either given the ultimate sacrifice or has given many years of service and contributions to emergency medical services in our community.
