3. Statement of Evidence / No more than one to four A4pages. / Specific statements addressing each of the selection criteria, ensuring there is supporting evidence against the claims.
4. Teaching andLearning Insight / No more than one A4 page / The nominee is also required to provide a personal insight into teaching and learning. This insight can take the form of a very brief statement; a photo; quote (something that will fit onto an A4 sheet).
5. Three references /
  • Standard form as supplied on the website.
  • No more than one A4 page per reference.

  • One MUST be from the Head of the nominee's School.
