ICRI 2017 Chapter Awards Forms

The ICRI Chapter Awards program gives recognition to our chapters that, through their operations, programs and activities, display a dedication to excellence and a commitment to success. These submission forms have been designed to accomplish two goals:

1. Provide a more thorough and useful annual report of the activities of your chapter.

2. Provide chapters a simple tool for self-assessment.

Do not view this document as merely a report form. Think of it as your “Roadmap to Success.” By reviewing your scores, you can easily pinpoint areas that may require more of your attention. It can also help you to identify the areas in which you excel. This can give you a clear picture of which programs are working and which should be targeted for improvement.

Through partnering with the ICRI office and other chapters, we can use this information to strengthen not only your chapter, but every chapter. A strong chapter enhances the value of membership, elevates the local community and furthers the mission of the Institute.

The levels of recognition you can achieve through the ICRI Chapter Awards are Chapter of the Year, Excellent Chapter and Outstanding Achievement.

Points

Point totals are based on the answers in the evaluation questions. There are a total of 211 possible points in the 2017 Chapter Awards.

Award Distribution

Chapter awards are distributed as follows:

Chapter of the Year: Highest point total of all chapters.

Outstanding Chapter:100 points or greater.

Excellent Chapter:76 – 100 points.

Deadline

The deadline for submitting the 2017 Chapter Awards forms is February 15, 2018.

Chapter Annual Reports

Please note that although you are not required to complete the Chapter Awards forms, every chapter must complete and submit the 2017 Chapter Annual Report form and submit it no later than February 15, 2018. Failure to do so may result in your dues reimbursement checks being withheld.

These operations represent the minimum activity level required of a chartered chapter.

AWARD FOR SPECIAL CHAPTER EVENT OR PROGRAM

PURPOSE: To recognize significant Chapter Events or Programs completed during the past year that served to benefit the community, or elevate awareness of the concrete repair industry and ICRI. (NOTE: For 2018, events or programs may have been completed in either 2016 or 2017 and not have been previously submitted.)

SUBMISSION REQUIREMENTS:Entries must be received from an ICRI Chapter in conjunction with the submission of the Annual Project Award forms or Annual Report form. In electronic format, provide an essay (500 word maximum, MS Word) describing the event or program and how it benefited the community, or elevated awareness of the concrete repair industry and ICRI. Provide up to five (5) photographs (minimum 400 KB, .jpg format) of the event or program, with brief captions. One of the five photographs shall be designated as the award photo (landscape format, not portrait), which will be used if the submission wins an award.

JUDGING:Voting members of ICRI’s Chapters Committee who are not members of a submitting Chapter shall judge the entries. Awards shall be announced at the ICRI Spring Convention.

DEADLINE:Entries must be received electronically by ICRI by 5:00 PM Central Standard Time on Thursday, February 15, 2018.
