£20,953 - £23,665 FTE based on 37.5 hours per week

JOB SUMMARY:
The role includes the assessment of nursing needs and the development, implementation and evaluation of effective programmes of care, maintaining and promoting standards. This post will involve taking charge of the unit on some occasions, directly assisting in the provision of a high quality clinical and organisational service. This will involve dealing with complex situations and having to take appropriate action in emergency and unforeseen circumstances. The post holder will be expected to work alongside and support non-registered nurses and student nurses.

Develop and maintain communication with people on complex matters, issues and ideas/in complex situations.
  1. Communication is a fundamental process to ensure continuity and high standards of care. It involves sharing information, ideas and feedback in order to empower patients and members of the multi-disciplinary team
  2. Has the skill in communication with a range of people on a daily basis and some who may have communication barriers
  3. To assist in providing feedback to other members on their communication at appropriate times
  4. Takes a proactive role in producing accurate and complete records of activities, communicate consistently with Hospice policies, procedures and legislation.

Promote, monitor and maintain best practice in health, safety and security.
  1. The post holder is required to familiarise him/herself with and comply with the Hospice policies and procedures
  2. The post holder must at all times respect patient confidentiality and most importantly the confidentiality of electronically stored personal data in line with the requirements of the Data Protection Act
  3. Creates and takes responsibility for a healthy and safe working environment.
  4. The post holder must be aware of individual responsibilities under the Health and Safety at Work Act and identify and report as necessary any untoward incident, accident or potentially hazardous environment

  7. Ensuring that all departmental resources, including drugs, are stored safely and used effectively and economically

  1. The post holder must at all times work in accordance with the NMC Code of Professional Conduct (2002)
  2. Demonstrate accountability for clinical practice and ensure competence in all areas of practice.

  1. Encourage patients to be involved in their care activities and make their own decisions wherever possible
  2. With guidance, assess, plan, implement and evaluate care for patients with complex needs and make change as appropriate and liaise with the multi-disciplinary team.
  3. Ensure documentation is accurate and up to date.
  4. Educate and support more junior staff in the assessment process.
  5. Safely administer prescribed medication and monitor effect.

  7. Ensure patients views are taken into account in the decision making process.

  1. The post holder plans their workload, using guidance, policies in accordance with their professional code of conduct, and in accordance to the needs of the patient group.
  2. The post holder is required to use their own judgement, to define day to day work priorities
and initiate treatments affecting patient care.
  1. On the whole, this post holder will be supported and supervised by more senior nurses on a daily basis.

Gather, analyse and report a limited range of data information.
  1. Demonstrate knowledge and understanding of research and development and how this influences nursing practice.
  2. Record patient data in the correct form and manner, whether paper or electronically. Assist in the development of electronic records and system.

  1. Provide formal teaching sessions to staff within the Hospice, as required.

Effort and Environment

Physical Effort
  • The post holder will have direct patient contact, delivering direct care within the in-patient unit, to meet the patient’s needs. There may be a requirement on occasions to work within day care to meet the needs of the organisation.
Mental Effort
  • Responsible for producing accurate patient records.
  • Responsible for calculating drug dosages, and administering medicines.
Emotional Effort
  • The post holder is required to support a caseload of patients, through a range of treatments and experiences. Many of which can be highly distressing, challenging and complex. The post holder will also support junior staff in the process.
Working Conditions
  • The post holder will work within the in-patient unit to deliver care. They are required to maintain close patient contact, which includes the handling of bodily fluids.
