Note: This waiver must be read and signed before the volleyball players listed below are allowed to participate in practice/warm-up, any competition or use of the volleyball courts.

I, the participant, affirm that I am eighteen years of age or older, have read this document and understand it contents. I acknowledge that volleyball or any sporting event is an extreme test of a person’s physical and mental limits and carries with it the potential of death, serious injury or property loss. I also fully understand that playing volleyball under the influence of alcohol and/or drugs is inherently dangerous.With a full understanding of the potential risks, I hereby assume the potential risks; I hereby assume the risks of participating.

I hereby take the following action for myself. I waive, release, and discharge from any and all claims or liabilities of death or personal injury or damages of any kind to person or property, which arise out of or are related to my participation in volleyball, athletic or sporting activities, the following entities:Bluemound Land Holdings LLC., Bluemound Lanes Inc., New Berlin Entertainment Center, New Berlin Ale House, New Berlin Ale House Volleyball, New Berlin Banquet Center, New Berlin Bowl and all of their employees, directors, representatives, share holders, officers, members, and their assigns. I agree not to sue any of the above mentioned for any of the claims or liabilities that have been waived, released or discharged herein. In addition to the foregoing I also understand that if Iam under the influence of alcohol or drugs before or during participating in volleyball, athletic or sporting events I waive, release and discharge any and all claims or liabilities that result from my actions against the persons and entities mentioned above and their insurance companies.I also indemnify and hold harmless the persons and entities mentioned above and their insurance companies from any claims made or liabilities assessed against them as a result of my actions or the actions of others that arise out of or are related to my participation in volleyball, athletic or sporting activities.
