KK RFV Entry Checklist
1. did you include all membership forms needed? (only needed once)

☐ / A copy of the member’s birth certificate to verify correct age group
☐ / A headshot photo of the contestant with a model release contract
☐ / A check or money order for the total amount, or a copy of your online payment receipt
☐ / A short biography for use by announcers, website administrators and in social media posts
2. Entering the race
☐ / Did you enter the open race? Make a note on your KK RFV entry form☐ / Did you include your KK RFV membership number on your race entry?
☐ / Did you include payment or a copy of your online receipt?
☐ / Did you use your horse’s registered name, or the grade horse’s name with your last name?
This helps for equi-stat reporting data
3. Packing for the Trip
☐ / Make a list of the specific items of clothing you’ll need to pack for your trip – REMEMBER – This is a full dress code race! Cowboy hats or western helmets must be worn. Long sleeve, collared shirts, jeans and western boots must be worn.☐ / Did you check the rules to ensure your equipment is within the guidelines? ☐ / Did you bring enough shavings/bedding, hay, feed, water buckets, etc.? Or did you ensure that you could purchase these items at the race?
