Dear Parent/Guardian,
As you might already know, your son has expressed interest in participating in the upcoming Denver Metro Area Charter School Basketball League. The league will be held at East Middle School (1275 Fraser St, Aurora, CO 80011) & South Middle School (12310 E Parkview Dr, Aurora, CO 80011). The first series of games will be held on November 17, 2017 which is our pre-season Tournament. The league will be held on Fridays starting on December 1st 2017 and ending on February 27th 2018. Each team will have one (1) league game scheduled per week. This waiver is required prior to the first game.
Your support and attendance is strongly encouraged.
Please understand that players and/or parents will be responsible for their own transportation to and from these scheduled events as the league or its officials will not provide transportation. By signing below you grant permission for your student to participate in the basketball league. Furthermore, your signature confirms that you acknowledge and agree that neither the league nor its officials assume no responsibility or liability should your player be injured or death occurs while traveling to and/or from or while participating in league activities.

Como usted ya sabe, su hijo ha expresado su interés en participar en la próxima temporada de baloncesto All-City Urban League. La liga tendrá lugar en la escuela East Middle ubicada en: 1275 Fraser St. Aurora 80011 y en la escuela South Middle ubicada en: 12310 E. Parkview Dr. Aurora, CO 80011. La primera serie de juegos se llevará a cabo el viernes 17 noviembre, que es nuestro torneo de pretemporada. La serie de juegos regulares se llevara a los viernes por la noche a partir del 1 diciembre, 2017 y concluye el 27 de febrero 2018. Cada equipo tendrá al menos un (1) partido de liga programado por semana. Esta exención es necesaria antes del primer juego.Su apoyo y asistencia es altamente recomendable.
Por favor, comprenda que los jugadores y / o los padres serán responsables de su propio transporte hacia y desde estos eventos programados como la liga o sus funcionarios. No se proporcionará transporte. Al firmar a continuación le concede permiso para que su estudiante participe en la liga de fútbol.
