Dear Parent,

To prepare for the 2014-2015 school year the following information needs to be provided to the school health office no later than August 20, 2014

1. The Medical Provider Order Form is required and necessary for the District Registered Nurse to develop a health care plan for diabetes management for your child at school.

Your child’s health care provider needs to give you new written orders for the 2014-2015 school year during one of your routine visits over the summer.

Based upon these orders I update your child’s health plan to insure all diabetes care needs are met while at school. I will contact you for questions and/or send a draft home before finalizing the plan. If there are any significant changes in your child’s diabetes management over the summer please leave me a message at the phone number below prior to the first day of school. Some changes in management such as insulin injections, insulin pumps or glucagon require new training and delegation of school staff.

In cooperation with the Colorado Department of Education Diabetes Resource Nurse Collaborative and the Barbara Davis Center the following process will continue.

  1. Insulin dosages for your child will be administered per the written physician’s order provided by parents. Any insulin dosage adjustments needed during the school day will require authorization by the district registered nurse per the physician’s order on file in collaboration with the parent. These changes are meant to be long term adjustments and not daily dosage changes.
  1. In the event your child requires a one-time insulin dosage change please contact the district registered nurse for your child’s school. School health aides are not able to change insulin dosages per instructions by parents over the phone. School health aides will only administer or confirm insulin dosages for your child based upon the written physicians order and as delegated by the district registered nurse.

2. The Student “Self-management Contract and Health Plan” is required if your child will be independently managing all aspects of diabetes care at school. This is typically used for High School and on occasion Middle School students. I have included this contract if your child currently is on a self-management plan or if I think it would be suitable for the upcoming school year.

If your child manages independently we recommend you continue to provide emergency back- up supplies for the school health office – this may include testing equipment, snacks and/or items needed to treat hypoglycemia.

3. The Diabetes Supply List – please review for required items needed at school.

As a reminder – IT IS THE PARENT’S RESPONSIBILITY TO PROVIDE ALL DIABETES SUPPLIES FOR THEIR CHILD. This includes all glucose testing supplies, juices, snacks and medications.

Be sure your child has adequate supplies and snacks required for daily diabetes management in the health office no later than August 20th and replace as needed throughout the year.

Please return all appropriate forms and give supplies to the school health aide prior to the first day of school – this will enable prompt attention to your child’s health needs.
