WHAT TO SUBMIT

1. Project Submission Form

This submission form must be fully completed. Please include details of the information you gathered, all of the actions your team took and your learning outcomes. In the form, the following must be filled out:

·  Guardian Consent Form

The signature of a teacher/youth leader/parent/guardian is required for young people under 18 years of age.

·  Participants List

Please ensure all names are spelt correctly as these names will be used on certificates and publications.

·  Project Summary

We will use this in any future publicity material including the website and the People’s Choice Award.

·  Teacher/Youth Leader Evaluation Sheet

Information contained in this sheet will be considered in the judging process.

2. Young Person Evaluation Sheet (*attached separately)

Please note every team member needs to complete this sheet. Information contained in this sheet will be considered in the judging process.

3. Any Visual Record of Your Work

You should show evidence of team work, research, action and engagement at all 6 steps of the project. Please submit any visual record of the work your team has done for your project along with the relevant evidence of the action such as scrapbook, PowerPoint presentation, photos, videos, song, website, etc. Be as creative as you like!

HOW TO SUBMIT

·  By 5pm on 24 Feb 2017, your 1) project submission form 2) young person evaluation sheets and 3) any visual record of your work must be posted to:

·  All your submission materials must be labelled with your registration number.

·  Project Submission Form: Please type your answers to each question in the boxes provided on the submission form and expand the boxes to accommodate your answers. Once completed, please print it for submission.

·  Young Person Evaluation Sheet (*attached separately)

Please note every team member needs to complete this sheet so you will need to print out these two pages (print on both sides) give one copy to each team member to fill out.

·  A Visual Record of Your Work:

·  Any 3D pieces, large posters, art/fashion pieces should be photographed and only the photos of these pieces (or links where they are uploaded) need to be submitted.

·  For any photos, videos, news articles and social media campaigns, please use a media sharing site such as Google Drive, Flickr and YouTube and include the links on your submission form.

·  Note: Once submitted, projects cannot be returned to entrants. Groups are advised to keep copies of all project materials for their presentation at the ECO-Dens and the Showcase and Awards Ceremony. Also, ECO-UNESCO takes no responsibility for items lost or damaged in the post. We recommend that you send by registered post (as you can track the status) or that you hand-deliver to our office if convenient. Make at least one copy of all project items to safeguard against all eventualities. Proof of postage is not proof of receipt. GUARDIAN CONSENT FORM

The signature of a parent or guardian is required for young people under 18 years of age. If participating as part of a group/school/youth organisation, by signing here the teacher/youth leader is confirming that they have obtained parental permission and are acting as a guardian. You do not need to send us individual consent forms for each young person.

Data Protection and Privacy
We will not share the entrant details with any third party. These details will be maintained within our participant files and we may contact you in the future about other ECO-UNESCO services. All participants are signed up to our monthly e-bulletin newsletter. You may unsubscribe at any time.
Policy on the Use of Photographs and Video
ECO-UNESCO may use any photographs, videos, sound, written or other materials as we see fit and without restriction.
Medical Assistance
By signing this form, you are authorising ECO-UNESCO to undertake appropriate first aid assistance in case of injury, accident or medical emergency.
