 / Transportation and Sustainabillity Survey--Spring 2010 / 

You are invited to participate in a confidential survey being conducted by the University of Louisville’s Sustainability Council and Sustainable Urban Neighborhoods Program. This survey is being sent to a random sample of UofL employees and students. Approximately 9,632 individuals will receive the survey. The purpose of this survey is to assess ways to reduce the number of car trips made to and from campus. The survey will take approximately 12 minutes to complete. All participants will have the option of entering a drawing for prizes, including gifts from a local bike shop, a local outdoor goods shop, a local bakery, and tickets to a UofL theater performance. We want to know what you think!
To participate, you must be 18 years of age or older and currently employed or enrolled at UofL. Participation is voluntary. Your decision not to participate or fully complete the survey will not affect any benefits to which you may be entitled from UofL. UofL Human Subjects guidelines guarantee your responses will be kept confidential. The information collected may not benefit you directly; however, the information learned may be helpful to others. At the end of the survey, you will see a debriefing statement with contact information of the investigator, and campus and community counseling services. Should you have any questions or concerns regarding this survey, please contact: Dr. John I. Gilderbloom, Director, Sustainable Urban Neighborhoods, 426 W. Bloom St., University of Louisville, Louisville, KY 40208, (502-852-8557).
