Application to Officiate Regionaland State Basketball Tournaments
If you wish to be considered to officiate a regional and a state basketball tournament, please complete this form and return to the WHSAA office, 6571 E. 2nd St., Casper, Wyoming 82609. This form MUST be returned no later than November 30, 2017 if you want to be on the ballot to be considered for tournament selection.
INSTRUCTIONS:
1.You must be a WHSAA certified official.
2. Regional Tournaments: If you wish to be considered to officiate a regional, please check your preference. The dates of the tournaments are given. Place a check mark by the dates when you are available.
3.State Tournaments: Please check which state tournament weekend(s) you would be available. Officials will only be assigned to one weekend.
4.Conflict of Interest: Please name any high school where you may have a conflict of interest or any high school which you prefer to avoid. Examples: A high school where you have taught, attended school, a relative teaches, a close friend coaches, relatives live in the community, etc.
5.Please accurately complete your 2017-18 basketball officiating schedule (those that you know at
this time). Give name of partner and indicate if the game is a boys’ or girls’ game.
6.If your schedule changes from what you turn into the WHSAA office, it is your responsibility to notify us via PHONE 307-577-0614 or E-MAIL: of those changes. Failure to do so could affect your post-season opportunities.
