“WORKING WITH NATURE” : JULY 4th STREET FAIR BOOTH

I have read and agree to the conditions set forth in the Call for Entries for the exhibition I am entering; and those enumerated herein. Permission is granted to ARTS FOR EARTH FOUNDATION (AFEF) to photograph any entry for educational, publicity, or promotional purposes on its web site or in other mediums. Submission of this form constitutes acceptance of all terms. AFEF reserves the right to make all final decisions regarding eligibility and exhibition of my artwork. I understand that the Entry Fees paid are non-refundable and that my artwork is MY responsibility to mount, maintain, promote to visitors andremove at the end of the show unless otherwise allowed by AFEF. I further agree to coordinate all art sales stemming from the show through the AFEF, and to donate a 30% commission on such sales at the date this form was filed. No artwork will be stored.

Neither the AFEF nor the Rincon-Vitobainsure artwork. I hereby release the AFEF and the Rincon=Vitoba and any of their agents, representatives, board members, elected officials, officers, employees and patrons from damage, theft or loss of any artwork currently on display on the grounds of the AFEF 4th of July Street Fair Booth, Main Street Ventura, CA. In addition, as an exhibitor, I hereby agree to defend, indemnify, and save harmless the Arts For Earth Foundation, City of Ventura, and the County of Ventura, including all of their boards, agencies, departments, officers, elected officials, employees, agents and volunteers, against any and all claims, lawsuits, demands, judgments, and all liability including, but not limited to, monetary or property damage, lost profit, theft, personal injury, wrongful death, general liability, automobile accidents, arising directly or indirectly from my participation in the exhibit sponsored or conducted in whole or in part by the Arts For Earth Foundation and Rincon-Vitoba..
