Worker Compensation, W-9, 1 million liability insurance, business license attached to bid.

All workers/companies are subject and required to meet TCA 49-5-406 regarding local, state and federal requirements for working on school properties and upon request must make back ground checks of any worker available to the Maintenance Director or provide signed/dated certificate affidavit with the bid.

Specifications of bid/ Bid form:

Scope: broom finish walks, flat work patio, footers, curbing and ditch paving.

All concrete shall include excavation to 8 inches and removal, fill and compacted of stone material of ¾ or smaller. Concrete shall be broom finish and 4000 psi concrete. Concrete shall have expansion joints worked in or sawn as requested by the Maintenance director based on the scope of the job. (Sidewalk, basketball court, or walkways.)

Upon finish:

Concrete work shall be to grade and not protruding above the ground as to present trip, or mowing hazards to people or equipment. All concrete poured against buildings shall have a composite barrier to prevent bonding of concrete to brick or foundation of building edge.

There shall be not splatter, rock, debris, form lumber, holes, deep equipment tracks left in surround area after a finish pour. Any damage shall be repaired, rock free, and seeded with straw to acceptable restoration of damaged area.

Curbing:

Footers and Foundations:

Footers to have minimum of 2 pcs of #4 rebar in each foot poured at 3 inches off bottom of trench. Footings to be 16 inches deep and concrete to be poured a minimum depth of 12 inches on turn down slab, and on 6 inches of gravel or crushed stone.
