I.   INTRODUCTION

A.   Purpose: To establish the guidelines for the daily/weekly maintenance and inspection of Las Vegas Fire and Rescue equipment and the recording of those activities.

B.   Scope: This procedure is provided for all Suppression and Support Services personnel to manage their assigned apparatus.

C.   Author: The Deputy Chief of Support, through the Deputy Fire Chief of Administration, shall be responsible for the content, revision and annual review of this information.

II.   RESPONSIBILITY

A.   Apparatus Engineer/Driver:

1.   Daily

a.   Reviews Form 22.58 “Daily/Weekly Apparatus Maintenance Schedule” and unit specific “Daily/Weekly Apparatus Maintenance Schedules (when applicable). (See Appendix I) to ascertain basic condition of apparatus.

Note: Unit specific Daily/Weekly Apparatus Maintenance Schedules apply to all engines, trucks, rescues, the Haz-Mat unit, and the Heavy Rescue unit.

b.   Performs all service and maintenance functions as outlined in this procedure using Form 22.58, “Daily/Weekly Apparatus Maintenance Schedule”, and unit specific Daily/Weekly Apparatus Maintenance Schedules (See Appendix I).

c.   Notifies the Company Officer of any item(s) discovered requiring immediate adjustment, replacement or repair by mechanics or technicians.

2.   Weekly

a.   This work shall be performed on the scheduled, weekly assigned apparatus maintenance day.

b.   Performs and records all service and maintenance functions as outlined in this procedure, using Form 22.58 “Daily/Weekly Apparatus Maintenance Schedule”, for recording the activity. (The servicing engineer shall sign his or her name in the appropriate box signifying he or she has completed and recorded the maintenance service). This will include informal inventory, and inspection of all allied equipment for proper marking.

c.   Items in need of repair shall be so indicated on the “Daily/Weekly Apparatus Maintenance Schedule”, Form 22.58 under “Remarks”. Items in the “Remarks” section shall be legibly written or printed.

d.   Items of repair or adjustment nature, requiring immediate shop attention, shall be reported to the Company Officer without delay.

B.   Company Officer:

1.   Reviews maintenance performance and recording of the activity.

2.   Reviews the immediate repair request of the Engineer and where determined valid, reports this via telephone to the Mechanical Division for their service.

3.   If safety is involved or a mechanical breakdown may ensue, due to the lack of immediate mechanical repair or adjustment, notify the Battalion Commander, who will make the decision if needed, to place the equipment out-of-service.

4.   After completion and signing of the form on Monday, the canary/pink copy of the Daily Weekly Apparatus Maintenance Form is to be sent to the Fire Equipment Mechanic Foreman. The originals shall be kept on the clipboard of the apparatus until the monthly inspection. Originals, after inspection, are to be kept in the station Master Apparatus files for one year.

C.   Battalion Commander:

1.   Reviews Apparatus Maintenance, Form 22.58, for correct preparation and to maintain an on-going awareness of apparatus condition.

a.   Will be involved in inspection of all apparatus.

D.   Mechanical Division Fire Equipment Mechanic Foreman or designee:

1.   Makes arrangement for any required adjustment or repairs to apparatus.

III.   POLICY

Each apparatus of this Department assigned to a Fire Station for emergency response, standby reserve or garaging will receive inspection and maintenance on a scheduled basis. This activity will be recorded and reported on standard forms approved by the Department, and will be monitored and reviewed by those persons assigned to that responsibility.

IV.   PROCEDURES

A.   Explanation – Completing Daily/Weekly Apparatus Maintenance Schedule:

1.   Monday date, include day, month and year.

2.   Unit running as (i.e. Engine 1), if Reserve unit, put “Reserve” as the company number. If the Reserve is running as an in-service unit, put the company number “Running as” in parenthesis. Example: Reserve (Engine 1).

3.   Permanently assigned unit number.

4.   Engineer/Driver signature that performed the checks.

5.   Daily Schedule: additional explanations:

a.   D-1 - Fuel level to be kept above 1 / 2 full, when practical. Dual tank units to keep a minimum of each tank half full.

b.   D-6 - Lights checked to include opticom and all running lights.

c.   D-8 - Visual check of gauges pertaining to the motor operation.

d.   D-9 - Any and all body damage, including bumpers, under frame and interior. Damage to be recorded on shift it occurred on or was found and followed up with the filing of an accident report.

e.   D-12 - Operate and lubricate as directed by the Mechanical Division.

f.   D-13 - Oxygen units to be checked as operational and bottle contents.

g.   D-16 - Air Pak checklist to be completed on this day.

6.   Every Monday Schedule: Additional Explanation.

a.   W-7 - Includes checking hydrometer reading, fluid level and terminals.

b.   W-19 - Confirms checklist was completed and assures full compliment of Air Paks are present.

7.   Please write/print legibly. All entries under remarks shall be explicitly identified (i.e. left front tore damaged).. Any fluid levels needing correcting are to be listed (i.e. added two quarts of motor oil).

8.   Signature of the Company Officer responsible for this unit on Monday. Signature confirms communication with the Engineer/Driver and that all deficiencies have been corrected or reported to the proper person or division.

9.   Those items given an (X) shall be identified under “Remarks”.

B.   Inspections – All apparatus will receive daily, weekly and monthly inspection and maintenance by the assigned Engineer/Driver. Battalion Commanders shall inspect apparatus on a monthly and at large basis. This inspection shall take place on a scheduled apparatus-cleaning day. Deputy Chiefs should involve themselves by periodically accompanying the Battalion Chief on these monthly inspections.

C.   Clipboard – All apparatus shall maintain a clipboard file containing a minimum of the following forms:

1.   Daily/Weekly Apparatus Maintenance Form 22.58.

2.   Apparatus Inventory Record Form.

D.   Apparatus Maintenance and Inspection Activities – A company level filing system regarding apparatus maintenance and inspection activities shall be established. This system shall have a separate file for each apparatus. The file shall be titled with the equipment number.

1.   Daily/Weekly Apparatus Maintenance Schedule Form 22.58 – Completed forms retained in station file for one calendar year. At the end of each calendar year, the complete file will be sent to the Chief of Support Services for proper disposal.

2.   Apparatus Inventory Record – Completed for all Station and Apparatus files. New forms can be printed monthly on the station computer.

3.   Change Authorization Form 22.60 – A completed copy shall be retained in the Station Apparatus file for the calendar year. An additional copy shall be sent to the Chief of Support Services.
