House Staff Responsibilities When Starting a New Rotation Post-Call or EC Post-Night Shift

Obviously, some house staff have to start a new rotation on a day following inpatient overnight call or a night shift in the emergency room. It is important for patient care that these transitions take place as seamlessly as possible. This situation is not addressed by the RRC in their regulation of work hours. Therefore, as a department, we have developed the guidelines below to optimize these transitions for both patients and the house staff.

House staff who are post-call, or who are coming off an EC night shift, when starting a new rotation should arrive at the new rotation as soon as possible for orientation. Ideally, the house officer should be able to go home immediately following orientation. This will be the case for all electives and all outpatient rotations. On most inpatient rotations, however, there usually will be patient-care responsibilities on the first day. In these cases, the patient load should be adjusted so that post-call residents can check out as soon as possible, which should be between 12-1pm, as determined by the time of arrival in the hospital the previous day. Total call time in the hospital should not exceed 30 hours.
