Wheaton Food Pantry Permission Form

Thank you for being willing to represent St. Alexander’s Faith Formation by serving at the Wheaton Food Pantry! (201 S. Naperville Road, Wheaton) Please fill out the attached form and return it to the Faith Formation Office. Transportation will be made on your own. You should meet in the parking lot of the Food Pantry at 5:20pm (10 minutes before you are to report inside to serve). Pick-up will be at 8:45pm in the parking lot of the Food Pantry. All Chaperones must have Protecting God’s Children certificate of completion in the office. If you have taken the class, please turn-in the certificate or proof that you took the class.
