2012 DALLAS COWBOYS

COMMUNITY QUARTERBACK AWARD

VOLUNTEER RECOGNITION & GRANT PROGRAM

What is the Dallas Cowboys Community Quarterback Award?

The Dallas Cowboys Community Quarterback Award is a volunteer recognition and grant program that salutes outstanding volunteers in the Dallas-Fort Worth metroplex. The Dallas Cowboys Community Quarterback Award honors individuals who exemplify leadership, dedication and a commitment to improving the community in which they live through volunteerism. Award winners receive grant money for their nominating organization. Since 2000, the Dallas Cowboys Community Quarterback Award program has disbursed more than $250,000 to local non-profits in honor of their outstanding volunteers.

Who is Eligible for the Dallas Cowboys Community Quarterback Award?

North Texas residents, age 13 years and over, who provide volunteer services for registered nonprofit 501c3 or 509 organizations are eligible for nomination. Nominees may ONLY serve in a volunteer capacity and must not receive any compensation — monetary or otherwise — for their efforts.

How do you nominate someone for the Community Quarterback Award?

Individuals may nominate themselves or be nominated by the nonprofit 501c3 or 509 organization which they serve by completing, in-full, the program nomination form (one set of forms for each volunteer). Organizations may nominate more than one volunteer for the award, as long as they submit a separate application packet for each individual nominee. Please note that only individual persons will be considered for the award. Groups of volunteers are not eligible.

When is the Dallas Cowboys Community Quarterback Award program run?

The Dallas Cowboys begin accepting nominations for the 2012 Community Quarterback Award starting Friday, October 5, 2012. Nominations must be submitted via postal mail or fax to the Dallas Cowboys Community Relations Department no later than FRIDAY, NOVEMBER 16, 2012.

How is the Dallas Cowboys Community Quarterback Award determined?

A review panel comprised of team, media and community representatives from the Dallas-Fort Worth area will review the nominations and chose a pool of 10 finalists for the Community Quarterback Award. The overall award “winner” will be chosen from this pool of finalists in late November.

What does the “winner” of the Community Quarterback Award receive?

Each of the 10 Finalists will receive a grant donated to his/her nominating nonprofit 501c3 or 509 organization from Gene and Jerry Jones Family Charities. From this pool there will be one “winner” chosen to receive $10,000 for his/her nominating charity, two “runners-up” selected to each receive $2,500 grants for their organization and seven “finalists” designated to have donations of $500 made on his/her behalf to the nominating non-profit agency.

Please feel free to add up to one (1) additional page of narrative or any other related materials (8.5 x 11 letter-size, non-stapled or bound, documents ONLY please) depicting any additional information you feel it is important for the review panel to know about the nominee.

Please note that the Dallas Cowboys are unable to returned submitted materials to the organization or nominee. For this reason, please submit 8.5 x 11 (letter size) photocopies of any additional documents or photos you wish to share with us.
