I have read and understand the payment rules and agree to pay for a minimum of 12 months. I have read RFU Declaration and section overleaf and have ticked any boxes I feel relevant.
Parents: Micros to u18 – I agree that my son/daughter may take part in the activities of the Club and that I am responsible for them immediately before and after every Club session (including training sessions, matches, social etc.). It is absolutely essential that at the end of the session someone is there to collect the children on time.
Parents: Micros to u11s – The Club does NOT operate a 'drop and go’ facility and would like parents/guardians/accompanying adults to stay, watch and encourage the children during the sessions. If a child has to be left for any reason the child's coach must be advised.
The club may wish to take photos or videos of the team or players solely for promotion and celebration of club and RFU activities and training purposes. You are required to confirm this statement. However, if you disagree with the above statement please email .

Other Important Notes
Whilst coaching and matches are supervised to minimise the risk of injury, rugby is a physical contact game. The RFU insurance scheme covers all paid up members of the Club, see http://www.rfu.com/ManagingRugby/Insurance/PlayersParentsAndGuardians.aspx but note that this is only basic cover and DOES NOT cover loss of earnings. The Club recommends that you consider taking out additional insurance.
Please note that the Club, its servants, agents or employees are not be responsible for supervising players before or after coaching sessions or matches. The RFU advises players to have an up to date tetanus injection. A properly fitted Gumshield is also recommended.
