Office Policies

Welcome to our office. We are pleased that you have chosen us for your orthopedic care.

Please take a moment to read through our office policies below:

Phone Calls: Our phones are answered from 8:30 AM to 5:30 PM on weekdays. Our office is closed for lunch 12:30pm -1:30pm. Routine questions such as scheduling appointments, refills, discussing billing questions or messages for the doctor or physician assistant are best handled during normal business hours. There will always be a physician or physician assistant available for emergencies who will be contacted by our answering service. If you have a life threatening emergency please call 911 or go to nearest emergency room right away.

Payment Policy: Payment is collected at the time services are rendered. Many insurance companies require a co-payment. We accept cash, checks and credit cards (Visa, Amex, MC)

Prescriptions:

Refill requests may be made during regular office hours. Some medications may require you to pick up the prescription from our office; they are available for pick up on Tuesdays and Thursdays only. Please ensure that all refills are requested at least72 hours prior to pick up days.

Controlled substance medications will not be prescribed prior to surgery.

In the event surgery has been recommended, this office will render the post-operative pain management. Narcotic pain management in the post-operative period may not exceed 6 weeks.

Under no circumstances will narcotic pain management be prescribed beyond a 90 day period. If required beyond 90 days, then a referral to a pain management specialist will be made.

For refills on most medications, follow up appointments will be required periodically.

Paperwork & X-ray Fees: There will be a $15.00 charge for completing any forms, (i.e. FMLA papers,disability forms,) as well as copies of X-rays. Please allow 5 business days for completion of forms.

Call Coverage:Due to Emergency Call Coverage, we occasionally have to schedule patients on an emergency basis. We apologize for any delay you may experience at your appointment time.

Medical Records:Should you need to request medical records; we require 48 hour notice for release of medical records upon completion of authorization form. Please be aware it may take 5 to 7 days, but we will make every effort to fulfill all requests as quickly as possible.
