Requirements Prior to Clinicals / Where to Locate Information: / Timeline:
Please submit the following:
Student Roster/List.
·  Verification that students/instructor having completed the criminal background checks with no results found.
·  Verification that students/instructors have been screened through OIG/SAM (the Office of Inspector General, and Excluded Parties Listing), with no results found.
·  Verification that students and instructor are in compliance with Health Care Requirements.
All of this information should be submitted on the Compliance Form. / Any positive background checks or names appearing on the OIG/SAM lists must be submitted to Aurora for approval to participate in the clinical experience. / These documents MUST be submitted to our office prior to the clinical experience to:

Obtaining computer ID and Password:
Nursing clinicals held at ASMC will need to document medications and nursing care in the Smart Chart Computer System. / In order for students/instructor to have access to the computer system, you must:
Utilize the Excel Template to request for computer ID’s and Passwords located on the website for Aurora Student Nursing ID and Password Section
Save the Excel Template to your computer.
Please provide all information requested on the Excel Template.
Attach and e-mail the completed Excel Template to us for processing at:

If information is received less than 3 weeks prior there may be delays in obtaining computer access.
Epic IDs will be distributed upon receipt of completed Orientation Checklist, Minimum Data Form, and Annual Confidentiality & Compliance Form.

Aurora Sinai Medical Center utilizes the Smart Chart Computer System for clinical documentation, result viewing, and medication administration. / The expectation is that you will be trained in the use of the system so you can train your students. Also, we expect schools to provide a representative for all “TBA” instructors, and that the representative trains the instructor when named.

Computer Training for Your Students
Please view the Smart Chart Documentation on the Student Nursing website for any updates and guidelines for your teaching.
In rare circumstances you can reserve ASMC’s Computer Training Room to train your students. The Computer Training Room at ASMC holds 13 computers. / Students should be trained on campus in the web-based application available at your school.
Instructions on how to access EPIC training environment on campus are housed at your college site

A copy of your course objectives is to be submitted to the Manager/CNS on your host unit. / Please complete and submit “Instructor Data Form” / Prior to start of clinical experience.
Acquaint yourself with the Charting System, Clinical Practice, and our Policies / Outlined on the “Orientation Checklist”. Policies can be found thru Dashboard on EPIC and EPIC training environment. / Hourly Rounding is a Mandatory Education and Skills that must be completed by all clinical students practicing at ASMC. Please contact Casie Arnone to schedule training for you and your group. This needs to be done within the first 2 weeks of the semester. Please contact or 414- 219-5486.
To arrange some clinical orientation days / Utilize the ASMC grid to contact the Manager/CNS on your host unit. / Prior to start of clinical experience.
Secure a password for Pyxis system. (Students will not be issued a Pyxis password).
Please review Pyxis Instructions prior to 2016 conversion / Report to inpatient pharmacy (located on the first floor ASMC) to obtain pyxis password. All pyxis users are required to sign the user agreement prior to obtaining pyxis access.
Before requesting medications from pharmacy, please check the electronic MAR (eMAR) to determine the location of the medication. Often, medications are in the unit based cabinets (UBC) or in the refrigerator as indicated in the comments section under the drug entry on the eMAR. / Prior to your clinical rotation.

If students sustains an injury while on hospital property / Refer to Student/Faculty Injury Policy
All student’s errors and incidents must be reported to the Unit Manager and an Incident Report must be completed. / An Incident Report should be completed for any unusual occurrence that:
Interrupts or interferes with providing a standard level of practice or patient care
Causes harm to any person or hospital property, or
Has the potential of causing harm (including needle/sharps punctures)
Badge for Secured Areas- / If you require access to secured areas please fill out the Faculty Badge Request form and submit to Jenni Cincotta at / Include list of areas that you need access to. If an OB instructor (returning or new) send recent photo to and Indicate college and hospital you will be teaching at.
Identification Name Badge. / An ID name badge should be provided by the student or the school. All students must wear an ID badge while participating in the clinical experience at the hospital. The ID should identify the individual as a “student”, along with first and last name.

Please note- ALL ITEMS on the Orientation Form should be reviewed by ALL before submitting electronic forms. This means DO NOT submit ANY forms until after you and your students have been on the unit and reviewed unit specific items- call lights, equipment, physical environment, etc. Computer training needs to be completed as well prior to submission of forms.
Instructors are required to submit electronic orientation forms as well as students. / These 3 documents MUST be submitted prior to patient care.
