Provider Patient Care Team Coordinator (Coordinator)
Job Type: Full-Time
The Coordinator is responsible for coordination of clinical and support services for program patients at the primary point of care. The Coordinator coordinates patient care among multidisciplinary clinical providers. Duties include but are not limited to scheduling program patient office visits, arranging for group visits and other related activities.

Review daily patient appointment logs and upcoming patient appointments and update records. Review patient needs with all levels of clinical staff and prepare for patients appointments in advance by following up on specific needs of individual patients.
Educate and inform patients on risk-reduction, disease management, self-management, risk-factor reduction, and screening for complications.
Assist patients with follow-up care, coordination of referrals to specialists and access to supportive clinical and community resources.
Develop outreach, education, and service opportunities for patients.
Coordinate patient shared medical appointments with primary care providers and specialists to ensure optimal clinical services during a single patient visit.
Collect and document clinical information in the electronic health record.
Assist in the identification and collection of key data points, factors and conditions, and tools for care coordination activities for identified health center patients.
Prepare educational materials for patients. Topics such as current protocols for disease management to be included and any other related information considered helpful for the patient.
Promote patient satisfaction and self-management by addressing patient concerns, and ensure that they have access to self-management education, and assessing collaborative goals and supportive care through community support.
Perform any other departmental or agency related duties or special projects as directed by Supervisor.
Requirements
Registered Nurse
Florida Licensed
Work effectively with multidisciplinary teams.
Work independently with minimum supervision.
Experience with disease management
Knowledge Microsoft Office.
Commitment to the mission of Camillus Health Concern, Inc.
Strong written, oral and Customer Service skills.
Bilingual (English/Spanish), Creole a plus.
