______I understand that participation in Dry Land and Ice Hockey activities involves a certain degree of risk and can be physically, mentally, and emotionally demanding. I have carefully considered the risk involved and have given consent for myself or my child to participate in this activity. I also understand that participation in this activity is entirely voluntary and requires participants to abide by applicable rules and standards of conduct. I assume all risks inherent and incidental to such participation and hereby release, indemnify and hold harmless USA Hockey, the Scotty Gomez Foundation Hockey Association, the Scotty Gomez Foundation, and its agents for any claim(s) arising out of injury to this Player. In addition, I hereby grant SGFHA/SGF permission to use my child in a photograph in any andall of its publications, including website and social media entries, without payment or any other consideration.
______In case of emergency involving my child, I understand every effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medication for my child. Medical providers are authorized to disclose to the adult in charge examination findings, test results, and treatment provided for purposes of medical evaluation of the participant, follow-up and communication with the participant’s parents or guardian, and/or determination of the participant’s ability to continue in the program activities.

HIGHSCHOOLSTUDENTELIGIBILITYREQUIREMENTS
Participationinhighschoolathleticsisaprivilege.Allstudentsareexpectedtocomplywithlocal,state,andfederallawsandtherulesandregulationsoftheAnchorageSchoolDistrict.Studentswillbesubjecttodenialoftheabilitytoparticipateiftheydonotmeeteligibilityrequirements,engageinbehaviorthatisdetrimentaltothewellbeingoftheteamorschool,areinviolationofthetobaccorule,areinviolationofthedrugandalcoholrule,orcommitcriminalactsasdefinedintheASDStatementofStudentRightsandResponsibilities.InallcasestheSuperintendentorhisdesigneeretainstherighttoreviewandreviseanydisciplinaryaction.
Astudentiseligibletoparticipateintheinterscholasticactivitiesofonlyonememberschoolperyear,exceptasprovidedundertheTransfer/ResidencyRule(Section9ofASAAHandbook).ThatschoolshallbeconsideredtheSchoolofEligibility.Astudentwhoissimultaneouslyenrolledatmorethanonememberschoolandwhowishestobeeligibletoparticipateintheinterscholasticactivitiesofoneofthoseschools,mustdesignatethatschoolasthe SchoolofEligibility.TheSchoolofEligibilitymustensurethatthestudentmeetsalleligibilityrequirementsbeforebeingpermittedtoparticipate.
A.Tobeeligibleduringaschoolsemesterforparticipationininterscholasticactivities,astudentmust:
1.Beproperlyregisteredina9-12highschoolprogramintheASD,anASDcharterschool,ASDalternativeschoolorprogram.
a.AstudentenrollinginanASDcharterschool,alternativeschoolorprogramduringthefirstsemesterofninthgrade;andastudenttransferringfromout-of-districtwhoattendsanASDcharterschool,alternativeschool or program after the first semester of the ninth grade, must designate a member school within thedistrictastheSchoolofEligibility.
b.TheSchoolofEligibilityofastudent,otherthanafirstsemesterfreshman,whotransferstoanASDcharterschool,alternativeschoolorprogramfromanotherschoolwithinthatdistrict,willbethememberschoolfromwhichthestudenttransferred.
c.AstudentattendinganASDschool,charterschool,alternativeschoolorprogramwhowishestochangehis/herSchoolofEligibilitywithoutacorrespondingparentmove,willbeineligibleforinterscholasticcompetitionatthenewSchoolofEligibilityfor180schooldays.
2.HaveaqualifyingG.P.A.:
a.Allfirstsemester freshmen are immediatelyeligibleatthebeginningoftheschoolyear.At theendofthe1stquarter,theymusthavea2.0GPAtoremaineligiblefortheremainderofthesemester.
b.Second-semesterfreshmen,allsophomores,alljuniors,andallseniorsmusthaveanoverall2.0 GPA dur-ingtheprevioussemesterandendoffirstandthirdquarters.Studentswhodonotmeetthe GPA portionofthisrequirementmayregaineligibilityduringthecurrentsemesterbyachievingandmaintaininganoverall2.0GPAattheendofthequarterorsemesterusingthegradesearnedduringthenine-weekgradingperiod.
c.Studentswhodidnotpass5classestheprevioussemestermayregaineligibilitybyretakingandcompletingthecoursefailedpriortothenextgradingperiod.Gradesarecheckedatthebeginningofeachfallsportsseasonandattheendofeachquarter/semester.Forpurposesofthissection,academicdeficienciesmaybemadeupthroughcorrespondencecoursesorsummerschool.(Correspondencecoursesmustbecompletedandpostmarkedpriortotheeligibilitycheck.Athletesmaybeginparticipationoncethefinalgradeisposted.)Gradesmustbepostedbythestartofthefirst(Fall)sportsseason.Thefirst(Fall)seasonbeginsthelastMondayinJulyorthefirstMondayofAugust.
3.Beenrolledintherequirednumberofsemesterclasses:
a.Allfreshmen,allsophomores,alljuniors,aswellasseniorswhoarenotontracktograduateand/orwhohavenotpassedallpartsoftheHighSchoolGraduationQualifyingExam(HSGQE)mustbeenrolledinaminimumoffive(5)semesterclassesthatleadtograntingofcredittowardgraduationfromtheschooldistrict.
b.Seniors who are on track to graduate and who have passed all parts of the HSGQE must be enrolled in aminimumoffour(4)semesterclassesinthedistrict.Seniorswhohaveaminimumof17.5creditsatthebeginningoftheirsenioryearandwhoenrollinaminimumoffour(4)semesterclassesthefirstsemesterwillbeeligibleduringthesecondsemesteraslongasenrolledinaminimumoffour(4)classesandenrolledinenoughclassestograduatesecondsemester.
c.For purposesofeligibility,"enrolled"meansthestudenthasregisteredandremainsinanapprovedcourserequiringregularattendanceand/orcoursework.Collegecoursesmaybeusedtodeterminethenumberof courses for enrollment if: 1) the student is currentlyenrolled in the course, and 2) the course has beenapprovedforASDcreditthroughCreditByChoiceProgram.Thefollowingunitsofcreditdonotqualifyforpurposesofdeterminingeligibility:highschoolcreditissuedformiddleschoolcourses;theWorldLanguagesIncentiveCredit;physicaleducationwaiver,andthecreditbyexamination.
d.KingCareerCentercourseswillbecountedasthree(3)semesterclasses.
B.Studenteligibilityischeckedonthefirstdayofthefallsportsseason.Eligibilitywillbecheckedagain,thesecondMondayofeachsubsequentquarter.Studentswhowerenoteligibleduringapreviousquarter/semester,maypractice,butnot play, afterschoolonthelastdayofthequarter,pendingthenewfindingsofeligibility.
C.Students who do not meet the eligibility rules of ASD and ASAA to participate in interscholastic activities duetoacademicdeficiencymayhavetheabilitytopracticewiththeteamfortheremainderofthequarteraslongasalloftherequiredpaperworkissubmittedtotheActivitiesoffice.Thestudentwillbeabletopracticeonly,theywillnotbeissuedauniformtoplayinacontestuntiltheendofthenextgradingperiod.Afterthenextgrading period,eligibilitymayberegained.
INORDERTO PARTICIPATE:
1.Aphysician’sclearancetoplaymustbeonfileattheschool.Thisclearanceisgoodforone year butmustnotexpireduringtheseasonofthesportinwhichthestudentisparticipating.
2.AHighSchoolAthleticActivityParticipationformmustbesignedandonfile,andparticipationguidelinesmustbeacknowledged.
3.StudentsandparentsmustcomplywiththeASAApre-participationguidelines.
4.AllrecordsmustbeinorderandformsmustbeonfileintheActivityOfficebeforethestudentiseligibletoparticipateinanyathleticprograms.
5.StudentspayanactivityfeeperASAAsportorapplyforafeewaiverbasedonfinancialhardshipthroughtheschoolactivityoffice.
6.a. Allplayersmusthaveten(10)separatedaysofphysicalpracticeinthesamesportactivitypriortothefirstdayofcompetition.
b.Whenastudentisparticipatinginarecognizedhighschoolsportactivitythatoverlapsthebeginningofanotherrecognizedsportseason,theAASAExecutiveDirectormaywaiveuptofive(5)practices.
c.Ifastudenthascompletedtherequiredpracticesbuthasnotcompetedforwhateverreasonforlessthantwoweeks,noadditionalpracticesarerequiredbeforereturningtocompetitionwiththeconcurrenceofthecoach.Ifastudentmissesbetween2and4weeksofpracticeandcompetition,5additionaldaysofpracticeandthe concurrence ofthecoach are required before returning tocompetition. If more than4 weeks havebeenmissed,thestudentmusthaveten(10)additionaldaysofpracticeandconcurrenceofthecoachbeforereturningtocompetition.
d.Memberschoolspermittingastudenttoparticipateininterscholasticcompetitionwithoutmeetingthepractice requirements of this section will be considered to be using an ineligible player and will be subjecttopenaltyunderArticle12,Section1oftheASAAhandbook. / e.ASAAprohibitsstudentsfromcompetingasamemberofanon-schoolcomp.orjuniorhockeyteambeginningwiththefirstdayofhighschoolpracticeuntilaschoolteamhascompleteditsseason.Theprohibitiononcompetingonanon-schoolteamwillbeliftedduring Thanksgivingandwinterbreakvacations.Althoughthisruledoesnotrestrictpracticeonanon-schoolteamduringthehighschoolseason,nostudentmayparticipateinahighschoolgameonadayinwhichhe/shehasparticipatedina“comp”practice.
ConditionsofStudentParticipation
A.ELIGIBILITYRULES
1.AthletesmustmeetalleligibilityrequirementsoftheAnchorageSchooldistrictandtheAlaskaStateActivitiesAssociation(ASAA).
2.Violationoftheeligibilityrulesshallresultindenialofparticipation.
B.DENIALOFPARTICIPATION
1.Anathletewhoisremovedfromateamfordisciplinaryreasonswillnotbeeligibletopracticeorplayanothersportduringtheseasonofthatsport.
2.Anathletewhoissuspendedfromtheregularschoolprogramforanyreasonwillnotbeeligibletopracticeorplayanysportduringtheperiodofsuspension.Asuspensionendsatmidnightofthefinaldayofthe suspension.
3.AnysanctionimposedundertheseConditionsofParticipationisseparateanddistinctfromanysanctionwhichmaybeproposedforviolationofanyotherschooldisciplinaryrequirement.
4.During anappeal process thestudentshallnotparticipateinthegroup’sactivitiesuntiltheappeal process iscompleted.
C.DENIALOFPARTICIPATIONFROMTEAMACTIVITIESFORSPECIFICCIRCUMSTANCES
1.Astudentathletewhoistruantforoneormoreclasses,orwhoisinattendanceforlessthanhalfofhisorherscheduledclassesduringschooldaycannotpracticeorplayonthatdaywithoutprincipalpermission.Studentsinviolationwillbesuspendedfortwogamesonthefirstoffense,removedfromtheteamonthesecondoffense.
2.Equipmentissuedtoanathleteishis/herresponsibilityforreturnorreplacement.Iftheequipmentisnotreturned,replacedorpaidfor,nolettershallbeawardednorshallthestudentbepermittedtoparticipateinanysport.
3.Denialfromparticipatingwiththeteamforaperiodoftimetobedeterminedbycoach/principal/andDistrictadministratorsmayresultfromthefollowing:
a.Insubordination
b.Obscenegestures;swearing
c.Provocation
d.Fighting
e.Stealing/Theft
f.Hazing/Initiation
g.Otherdisciplinarysituationswhichmayarise
4.Withadministrativeapproval,thecoachmayestablishadditionalparticipationrules.
D.ThecompleteTobacco,AlcoholandControlledSubstances(TAD)Polityfollows.
Timeperiodduringwhichpolicyapplies
Thepolicyinthissectionappliestoanystudentwhoisparticipatingorhasparticipatedininterscholastic activitiesstartingfromthestudents'firstparticipationininterscholasticactivities,includingformalpracticeswhichprecedeinterscholasticcompetitionaftertheinitialsigningoftheStudent/Parent/LegalGuardian(TAD)AcknowledgementForm,atanyASAAmemberschool,andcontinuinguntilthestudentgraduatesfromhighschool.
StudentssanctionedundertheTAD guidelinesmustcompleteASAAeducationcomponentbeforereturningto play.Violationsofthispolicyarecumulativeandprogressivethroughoutastudent'shighschoolyears.
TOBACCORULES-Violationsoftobaccorulescumulativethroughoutschoolyear.Studentswhoviolatethetobaccoruleswillbesubjecttothefollowingsanctions:
a.FIRSTOFFENSEFORPOSSESSIONORUSE-Suspendedfrominterscholasticactivitiesandpracticesfor10calendardays.Fiftypercentofthesuspensionwillbeforgivenandthestudentmayreturntopracticeifthestudentandparent/guardiancompletetheFirstOffenseeducationalcomponent.
b.SECONDOFFENSEFORPOSSESSIONORUSE-Lossofpracticeandathleticparticipationforforty-fivecalendardays.
c.StudentswhoviolatethetobaccoruleswhileunderschooljurisdictionaresubjecttosuspensioninaccordancewithAnchorageSchoolDistrictregulationsandcannotparticipateduringthelengthofthesuspension.
E.DRUG/ALCOHOLRULES-
Theactualorattemptedsaleof,useof,possessionoftobacco,alcohol,inhalants,illegaldrugs,substancesdesignedtolooklikeillegaldrugs,substancespurportedtobeillegaldrugs,ordrugparaphernaliashallsubjecttheathlete tolossofpracticeandathleticparticipation.
UnderSchoolJurisdiction
Studentswhoviolatethedrug/alcoholruleswillbesubjectto:
a.FIRSTOFFENSEFORPOSSESSIONORUSE-SuspensionorexpulsioninaccordancewithAnchorageSchoolDistrictRegulations.Lossofpracticeandathleticparticipationduringthesuspensionorexpulsion,orfortencalendardays,whicheverisgreater.Fiftypercentofthesuspensionfromathleticparticipationwillbeforgivenandthestudentmayreturntopracticeifthestudentandparent/guardiancompletetheASAAFirstOffenseeducationalcomponent.
b.SECONDOFFENSEFORPOSSESSIONORUSE,ORFIRSTOFFENSEFORSELLING,DISTRIBUTING,ORATTEMPTINGTOSELL-SuspensionorexpulsioninaccordancewithAnchorageSchool District Regulations -Lossofpracticeandathleticparticipationduringthesuspensionorexpulsion.
NotUnderSchoolJurisdiction
Studentswhoviolatethedrug/alcoholruleswhilenotunderschooljurisdiction:
a.FIRSTOFFENSEFORPOSSESSIONORUSE-Lossofpracticeandathleticparticipationforten calendar days. Fiftypercent of the suspension from athletic participation will be forgiven and the studentmayreturntopracticeifthestudentandparent/guardiancompletetheASAAFirstOffenseeducationalcomponent.
b.SECONDOFFENSEFORPOSSESSIONORUSE,ORFIRSTOFFENSEFORSELLING,DISTRIBUTING,ORATTEMPTINGTOSELL-Lossofpracticeandathleticparticipationforaminimumofforty-fivecalendardays.
c.THIRDOFFENSEFORPOSSESSIONORUSE,ORSECONDOFFENSEFORSELLING,DISTRIBUTINGORATTEMPTINGTOSELL-LossofeligibilityforthedurationofAnchorageSchoolDistrictattendance.
F.CRIMINALACTS
StudentswhocommitcriminalactsasdefinedintheASDStatementofStudent’sRightsandResponsibilitieswillberemovedfromtheteamfortheremainderoftheseason.
1.TheschoolwillconductaninvestigationindependentofthePolice.
2.Thestudentmayappealthefindingoftheinvestigationtotheschoolprincipal.
3.Thestudentshallnotparticipateintheteam’sactivitiesuntiltheappealprocessiscompleted.
