SUPPLY LIST AND INFORMATION
2015-2016
We will require a CERTIFICATION OF IMMUNIZATION COMPLIANCEmarked “complete forschool entry” for each pre-kindergarten student who is enrolled. This may be obtained from either your doctor or the health department. Also required is a copy ofthe student’s BIRTH CERTIFICATE and SOCIAL SECURITY CARD. These documents will be a part of the student’s permanent records and MUSTbe supplied to WS by “Meet the Teacher” morning.

2 boxes of wipes
2LARGE boxes of facial tissues
2 rolls of paper towels
1 box of CrayolaCrayons (Regular size-24 colors per box)
1 box of Crayola Crayons (Large size – 8 colors per box)
2 boxes of washable “CLASSIC”Crayola markers (10 colors per box)
1 Elmer’s School glue (4 oz. size) – Please DO NOT buy “no run” glue.
1 mat for rest period (child size, no adult exercise mats andno cloth mats! (NO BLANKETS OR
STUFFED TOYS)
1 box of quart size ziplock bagswith zipper.BOYS ONLY

1 pair safety scissors (children’s Fiskars recommended)
24glue sticks
$7.00 for R.A.M.S. notebook materials due at the “Meet the Teacher” morning.
1 LARGE backpack with WIDE opening (large enough for a 3-ring binder AND a lunch box to fit in)
1 can of Clorox or Lysol wipes
1 package cheap, large, white paper plates
*Please send supplies to school in a paper sack. There is NO need to label supplies with your child’s name. We group supplies and share.
LUNCH:
See R.A.M.S. notebook for monthly and ala carte menus with price lists.
SNACK:
We ask that parents supply our daily snack for a week at a time. We will work up a schedule and let you know when it is your turn. (Please, nothing elaborate, just something to tide the students over until lunch).
CLOTHES:
Please bring an extra set of summerANDwinter clothes for your child to the “Meet the Teacher” morning that will be kept at school to use in case of an accident. Place the clothes in a plastic gallon ziplock bag marked with your child’s name. Since these clothes will possibly be kept all the school year, send a size that can be worn in seven or eight months. Please include underwear and socks. This does not have to be a school uniform!
