2014-15 STUDENT AND ATHLETIC ACCIDENT INSURANCE PROGRAM

Dear Parents:

The Warren Hills Regional Board of Education is pleased to announce that, for the 2014-15 school year, we have purchased compulsory full-excess student and athletic accident insurance through Bollinger, Inc. This means that all students, athletes, band members and cheerleaders are automatically covered on a full-excess basis under this year’s program while participating in school-sponsored and supervised activities. There is no cost to parents for this coverage.

In the event that medical expenses are incurred as a result of accidental injuries, this accident policy provides “excess” coverage. The policy will pay benefits on a covered accident after any other insurance the student may have, including HMO’s. The primary purpose is to pay only the portion of medical expenses not covered by personal or group insurance that already covers the student. If there is no insurance in force, or if other insurance does not offer coverage for a particular accident, then this policy will pay benefits on a primary basis.

It is important to note, however, that parents may purchase the following optional student plans:

1. $500,000 Round-the-Clock coverage for non-school-sponsored activities at the cost of $84.00 per year.

2. $5,000 Dental Accident coverage for non-school sponsored activities at a cost of $20.00 per year.

3. $10,000 Student Term Life Insurance at a cost of $30.00 per year.

Those parents who wish to purchase the above-listed optional plans must obtain the appropriate application on line at complete the application, make a check or money order (no cash please) payable to Bollinger, Inc. and forward directly to:

On the site you may download & print Claim forms, enrollment forms & the above listed optional plans brochure. It is important to read and follow the instructions on the back of the Bollinger claim form. Injuries must be reported to Bollinger within ninety (90) days of the date of injury.
