Dear Parents/Guardians:
Kindergarten registration for the school year 2017-2018 will take place at Gayhead Elementary School on Tuesday - Thursday, March 21, 22, and 23. The hours of registration will be between 9:30 a.m..- 11:30 a.m. and 1:00 p.m. - 2:30 p.m.
When you come for your registration appointment, please have the registration packet completed and have your required documentation. Registration will take approximately 15 – 20 minutes. Scheduled appointment times will ensure time to go over questions. When you come to register, please remember to bring your photo id to gain admittance to our building.
The district is also providing an evening registration for parents who are unable to register their kindergartner during the daytime hours. That evening registration will be on Tuesday, March 21st and Wednesday, March 22nd from 5:30-8:30 at Myers Corners Elementary School, located at 156 Myers Corners Road, Wappingers Falls, NY 12590. You do not have to make an appointment for the evening registration.
Prior to coming to registration, please be sure the enclosed forms are completed in advance and brought to registration:
· REGISTRATION DATA SHEET: To be completed and signed by you (unless you are completing the online Pre-Registration). This information is required by New York State. At the bottom of the data sheet please list siblings or other children. Please remember to sign the form at the bottom right.
· HOME LANGUAGE QUESTIONNAIRE (HLQ): To be completed and signed by you. Kindly complete this form and check the appropriate boxes.
· KINDERGARTEN SOCIAL – HOME SURVEY
· HEALTH DATA SHEET: To be completed and signed by you.
· IMMUNIZATION REPORT: To be completed and signed by the doctor, clinic or nurse (or you may bring your child’s immunization record, signed by the doctor).
· HEALTH EXAMINATION RECORD: According to Education Law, Section #903, the mandated physical examination of students entering school must have been accomplished within the previous year. Physicals after September 6, 2017 will be accepted.
· DENTAL HEALTH CERTIFICATE: Recommended to be completed and signed by a dentist.
· MCKINNEY-VENTO ACT QUESTIONNAIRE (If Applicable)
· STUDENT INFORMATION PROFILE
The following information will be required at the time of registration:
· PROOF OF BIRTH/AGE: Examples: the ORIGINAL state-issued BIRTH CERTIFICATE with the raised embossed seal (not hospital birth certificate) or original Baptismal Certificate. A copy is not acceptable. If neither is available, a passport will suffice. Only children who will be 5 years old on or before December 1, 2017 are eligible for September 2017 admission. THE REGISTRATION CANNOT BE PROCESSED WITHOUT
THIS DOCUMENTATION.
· PROOF OF GUARDIANSHIP: This information is necessary in cases of divorce, Order of Custody, or when the student is not living with the natural parent. This may include legal documents which prove legal guardianship or parental custody; may include divorce papers, adoption papers, or court order.
· VERIFICATION OF RESIDENCY: TWO (2) PROOFS OF RESIDENCE MUST BE PRESENTED AT TIME OF REGISTRATION – Please refer to the Guidelines for registering your child in the registration packet for acceptable proofs of residency.
The child must be living at the address given at the time of registration. If the address where you reside is listed under someone else’s name, their two proofs of residency are required along with a Homeless Referral (McKinney-Vento) Form.
DAYCARE/BABYSITTER TRANSPORTATION APPLICATION forms are included in the registration packet for those requiring transportation to and/or from a licensed/registered daycare facility five days a week. Please
Refer to the WCSD Bus Transportation to and from a Babysitter or Daycare for the 2017-2018 School Year information in the registration packet.
Application for transportation to daycare must be filed with Transportation by April 1, 2017.
