Notification Requirements

  1. The Provider agrees that it will comply with the notification requirements for class transfers of children at the same provider location as follows:

1. When a child transfers from one VPK class to another VPK class within the same site with the same numberor differentnumberof VPK hours, the Provider will notify the Coalition in writingwithin fourteen (14) calendar days but is encouraged to notify the Coalition within two (2) business days, with the effective date and written parent consent. Provider is encouraged to notify the Coalition prior to implementing changes as changes may result in Provider’s failure to comply with all qualifications and requirements.

  1. The Provider agrees that it will comply with the notification requirements for changes to information provided on Forms OEL-VPK 10 and OEL-VPK 11 A and Bas follows:
  1. The Provider agrees that it will notify the Coalition in writingwithin fourteen (14)calendar days of any change of the information submitted on those forms. Provider is encouraged to notify the Coalition prior to implementing changes as changes may result in Provider’s failure to comply with all qualifications and requirements.
  1. The Provider agrees that it will comply with the notification requirements for changes to class calendars and for submitting documentation demonstrating temporary closure and subsequent reopening as follows:
  1. The Provider may modify its calendar up to two times for any reason other than a temporary closure caused by emergency circumstances.
  2. The Provider shall submit in writing to the Coalition an initial notification of the closure before the Coalition’s close of business on the first day of the closure. Initial notification shall indicate the Provider’s name and address, the dates(s) of the closure, the VPK class(es) affected by the closure, and, if available, an estimate of the date upon which the affected VPK class(es) will resume instruction.

b. The Provider shall submit in writing to the Coalition a notification of calendar modification no later than two (2) business days following resumption of VPK instruction. Notification of schedule modification and resumption of instruction shall indicate the Provider’s name and address, the class(es) resuming instruction, the dates(s) which each VPK class did not offer instruction as previously scheduled and a revised class calendar which indicates the date and times added to the schedule as revised to restore the hours lost as a result of closure.

Notification Requirements

  1. If the modification of the class calendar results from a temporary closure caused by emergency circumstances as described in 6M-8.204(5)(a), the Provider shall submit in writing to the Coalition a notification of calendar modification no later than two (2) business days following resumption of VPK instruction. Notification of schedule modification and resumption of instruction shall indicate the Provider’s name and address, the class(es) resuming instruction, the dates(s) which each VPK class did not offer instruction as previously scheduled, and a revised class calendar which indicates the date(s) and times added to the schedule as revised to restore the hours lost as a result of closure.
  2. The Provider shall revise its class calendar to restore VPK instructional days which are lost due to temporary closures caused by emergency circumstances in excess of a total of five (5) instructional days for a VPK class.
  3. A Provider may revise its class calendar as frequently as necessary to restore the instructional days lost due to a temporary closure caused by emergency circumstances.
  1. The Provider agrees that it will comply with the notification requirements for dismissal of students as follows:
  1. The Provider agrees to submit in writing to the Coalition documentation specifying reasons for removing the student from the class within fourteen (14) calendar days of the removal in accordance with the Coalition’s notification procedures.
