  1. OVERALL PURPOSE

These new roles have been developed within a partnership arrangement between Forth Valley Alcohol and Drug Partnership Servicesand Combat Stress. Combat Stress is the leading provider of mental health services to British Veterans arising from military service and is currently developing a pilot Substance Misuse Case Management Service for Veterans across the UK in partnership with local providers.

Forth Valley Alcohol and Drug Partnership services are committed to delivering a recovery oriented system of care throughout their substance misuse treatment system. This partnership allows the development of a specialist recovery pathway for Veterans experiencing substance misuse who may also have mental health problems. The aim is to use assertive outreach to engage Veterans in case management for specialist treatment within the Forth Valley area, and refer to other organisations to improve their health outcomes.

The post holder will promote a culture of engagement with service users that includes as core principles and values an approach that is kind, courteous, empathetic and respectful. The Veterans Substance MisuseCase ManagementNurse will promote the principles of self-care to service users, families and carers. They will empower them to access and use health resources in their community.

  1. KEY DUTIES AND RESPONSIBILITIES
  • To employ assertive outreach principles to engage Veterans with current histories of substance misuse and mental health problems.
  • Manage referrals via the Single Point of Referral (SPR) from a range of sources including general practitioners, community addiction staff, social work staff and voluntary sector agencies.
  • Undertake expert, evidence based assessment, care planning and treatment for Veterans with substance misuse problems.
  • Responsible for the care management of own caseload by assessing risk, evaluating the effectiveness of interventions and on-going care planning.
  • Ensure integrated care with secondary addiction services is achieved and developed on an on-going basis.
  • Work in collaboration with statutory and voluntary sector agencies to provide appropriate specialist addictions interventions for Veterans with substance misuse and mental health problems.
  • Promote and develop strong working partnerships between local substance misuse agencies, Combat Stress and other Veteran relevant organisations
  • Provide specialist advice and support to external treatment providers on issues related to the management of Veterans within external services.
  • Attend and contribute to meetings with key stakeholders to discuss case management issues.
  • Maintain high quality, contemporaneous written and electronic records to Nursing and Midwifery Council standards and provide written reports for services as required.
  • To adhere closely to confidentiality and data protection protocols in the communication of personal information between services.
  • The post holder is an autonomous practitioner and responsible for managing their own caseload.
  • The post holder may be directly responsible for the line management of staff/volunteers.
  • Contribute to specific “in-service” training programmes with a focus on Veteran related issues, particularly with regard to substance misuse.
  • Participate in the development of audit and research, particularly in relation to Veterans with substance misuse and mental health problems.
  • Able to work flexibly, including evenings and weekends, in accordance with the requirements of the Service.

Audit and Research

  • To plan and participate in audit of clinical practice and assist with data collection for research.
  • To implement audit and research findings to deliver evidence based practice.

Education and Training

  • To undertake mandatory training and appropriate training identified through supervision and appraisal.
  • To lead and participate in the development and delivery of training within the team and to other professionals and agencies.
  • To identify own training and developmental needs and participate in a personal development plan to meet identified needs.
  • To provide support, guidance and supervision to junior staff.
  • To provide mentorship to students on placement.

Professional responsibilities

  • To maintain registration with the NMC and follow their guidelines for conduct and professional practice.
To participate in regular clinical professional supervision and appraisal as consistent with the requirements of the Organisation
  • To maintain the confidentiality of all client data in line with Organisational policy and the Data Protection Act 1998.
  • To represent the Charity in a professional manner at all times.

  • To undertake any other duties at the request of the line manager which are commensurate with the role, including project work, internal rotation (as per contract) and absence cover.
  • To contribute to the maintenance of a healthy and safe working environment by adhering to Health and Safety Organisational policies.

5.RISK MANAGEMENT

All staff have a responsibility to manage risk within their sphere of responsibility. It is a statutory duty to take reasonable care of their own safety and the safety of others who may be affected by acts or omissions.

As an employee of Combat Stress you are required to be risk aware, readily able to identify risks faced by you and by Combat Stress in the course of your day-to-day employment. Where a new risk is identified it is to be reported through your line manager.
