Product/Service Description: The Vermont Department of Health’s Children with Special Health Needs (CSHN) program is seeking proposals from a highly qualified licensed psychologist to perform psychometric/adaptive functioning testing and coordination for the Department of Health involving the diagnosis of complex developmental conditions and special needs of children referred to the Child Development Clinic. Testing includes review of medical and educational records, determination of appropriate testing tools, assessment of cognitive and adaptive function levels, and scoring of test results. Contractor should have extensive experience with cognitive and adaptive standardized tests for the diagnosis of intellectual disability, ADHD and Autism Spectrum Disorders. These tests may include but are not limited to the Wechsler Intelligence Scale for Children-Revised (WISC-R), Wechsler Preschool and Primary Scale of Intelligence (WPPSI), Stanford Binet Intelligence Test, Vineland Adaptive Behavior Assessment, the Adaptive Behavior Assessment System (ABAS), and the Autism Diagnostic Observation Schedule (ADOS).

Anticipated contract period will be September 1, 2011 through August 31, 2012, with option to renew for two (2) consecutive one (1) year periods. Contractor must be available no less than twenty-four (24) clinic dates over the course of the contract period. Contractor will perform work at sites to be determined by mutual agreement, but which may include Rutland and White River Junction.

Contractor will:

§  Participate in an orientation of the VDH-CSHN Child Development Clinic.

§  Provide written reports of evaluations in a clear, concise, and timely manner for each child evaluated within the CSHN standard, which is within fourteen (14) days of the visit.

§  Participate as a member of an interdisciplinary team, consisting of developmental pediatrician, psychologist, and medical social worker. Interaction with both staff and clients may be on a one-to-one or group basis.

§  Translate observations into meaningful data to assist the interdisciplinary team through the diagnostic and interpretive process in determining a child’s strengths, needs, prognosis, and appropriate intervention strategies.

§  Consult with parents, educators, physicians, social workers, and others regarding test results and appropriate methods of providing assistance to individual children.

§  Serve as a resource person to educational, medical and other professional groups regarding psychometric assessment techniques, their strengths and limitations, and their use in developing total care plans for children.

§  Perform all VDH Child Development Clinic work in a manner which is family-centered, supportive, and non-judgmental and representative of Children with Special Health Needs and the Vermont Department of Health in the community.

§  Ensure flexibility and adaptability with regard to cultural competency and cultural intelligence in the provision of quality pediatric services for Vermont children. Contractor will recognize cultural competency/cultural intelligence is an on-going learning process and continuously evolves over time. Cross-cultural communication is essential in meeting our patient’s needs and is both verbal and non-verbal. It is expected that any person who has limited ability to read, write, speak, or understand English be provided with an interpreter.

§  Agree to see patients as they are scheduled by CSHN Regional Clinic Coordinators. A full clinic day, depending on patient availability, shall include a minimum of two new patient visits or three follow-up visits, or a combination of new and follow-up visits.

§  Participate in continuing education to maintain qualifications to administer appropriate developmental tests.

§  Seek prior authorization to attend any visits outside of the scheduled CDC clinic visits/evaluations from the CDC Clinic Coordinator.

§  Recognize the importance of early and open communication when being a member of a multidisciplinary team. Timely communication keeps scheduling conflicts at a minimum and maximizes support of CSHN goals and outcomes.

§  Follow appropriate HIPAA procedures and ensure that all e-mail with external recipients (outside State government) that contains confidential/protected information will be sent using the required [secure] system implemented in July 2009.

This contract will be subject to review throughout its entire term. The Contractor will be accountable to the CSHN Child Development Clinic Director and her designee(s), and is responsible for achieving project deliverables, adhering to an aggressive schedule and fulfilling contract provisions.

The State reserves the right to accept or reject any or all proposals. Selected State staff will evaluate proposals. If a proposal is selected, the chosen Contractor will be invited to negotiate a contract for the activities outlined in the request for proposal.

Special Instructions:

Prospective bidders should include a detailed CV. A minimum of a master’s degree in psychology is required. Licensure is also required. Bids should include an all–inclusive per evaluation rate (visit, scoring, interpretation, and written report). Please mark clearly on outside of envelope “Bid Proposal – Licensed Psychologist”. The closing date for the receipt of proposals is 4:30 PM EDT, June 30, 2011. Contractor must be able to provide proof of insurances as required by the State of Vermont. See attached Contract template, Attachment C.

U.S. Mail: Bidders are cautioned that it is their responsibility

to originate the mailing of bids in sufficient time to

insure receipt by the State prior to the closing of

bid request.

Express Delivery: If bids are being sent via an express delivery

service, please be certain that the RFP designation is clearly shown on the outside of the delivery envelope.

Hand Delivery: Hand carried bids must be delivered to a

representative of Children with Special Health

Needs if the Administrator is unavailable upon

delivery.
