UHCare Problem List Manager

This document provides step-by-step instructions for adding and updating diagnoses and problems in UHCare using Problem List Manager.

Accessing Problem List Manager

Problem List Manager can be accessed using the following methods in UHCare:

  1. From the Main Application toolbar, click Enter Problem List. .
  2. From a Structured Note, Assessment and Plan section, click Add Item .
  3. From the Admission Order Set, Admit order , click Admitting Dx.
  1. From the Discharge Order Set, Discharge order, click Discharge Dx.

Adding Diagnoses and Problems

Adding an Admitting Diagnosis

  1. From the Problem List Manager window, Select a Type section, click Admitting Dx.
  1. From the Browse tab, click the triangle next to the desired category folder.
  2. Click to open desired diagnosis folder.
  3. Click ADD to select and add desired diagnosis to the Problem List.

Only one admitting diagnosis can be entered and saved on the Problem List.

Adding Additional Diagnoses and Problems

  1. From the Problem List Manager window, Select a Type section, click the desired diagnosis type.
  2. Repeat steps 2-4 under the above section “Adding an Admitting Diagnosis”.

If you are adding diagnoses and problems from within an Admission or Discharge order set, the codes and associated descriptions will display on the Orders tab.

Discontinuing and Modifying Diagnoses and Problems

Discontinuing Diagnoses and Problems

  1. From the Problem List Manager window, Existing Problem List section, select desired diagnosis or problem.
  2. From the Existing Problem List section, Action List, click Discontinue.

Modifying Diagnoses and Problems

  1. From the Problem List Manager window, Existing Problem List section, select desired diagnosis or problem.
  2. From the Existing Problem List section, Action List, click Show/Modify.
  3. From the Problem List Details window, enter the onset date, expected resolution and change the problem status as desired –OR- enter a free text description.

Tips and Tricks

Sorting by Diagnosis Name

Diagnosis names can be sorted alphabetically to streamline your diagnosis name and code search.

  1. From the Problem List Manager window, Add New Problem List section, select the desired search tab, e.g., Favorites, Browse or Full Catalog Search.
  1. From the Browse tab, click the triangle next to the desired category folder.
  2. Click to open the desired diagnosis folder.
  3. From the Problem List grid, click the Name header:
  1. Click the Name header once to sort the diagnosis name codes in ascending alpha order.
  2. Click the Name header twiceto sort the diagnosis name codes in descending alpha order.

Searching for a Diagnosis within a Folder

Use the Find in folder function to search for diagnoses contained in category folders. Using this function eliminates the need to open a category or sub-category folder to initiate your search.

  1. From the Problem List Manager window, Add New Problem List section, select the desired search tab, e.g., Favorites, Browse or Full Catalog Search.
  1. From the Browse tab, click the triangle next to the desired category folder.
  2. In the Find in folder field, type the desired diagnosis name and click search .

The diagnosis code(s) populates the Problem List grid and is available to add to the Problem List.

Working with Favorites

Frequently used diagnoses and problems can be saved to the Favorites tab to streamline diagnosis/problem search and entry in Problem List Manager.

Adding Diagnoses and Problems

  1. From the Add New Problem List section, select the Browse tab.
  2. From the Browse by Category list, lick the triangle next to the desired category folder.
  1. Click to open desired diagnosis folder.
  2. Click to highlight the desired diagnosis.
  3. From the Add New Problem List Action List, click Add to Favorites.
  1. From the Favorite Problem List window, clickOK to add the diagnosis/problem to your favorites list.
  2. Click the Favorites tab to access your favorite diagnoses and problems.

Creating Favorite Categories

Favorite categories can be created to organize your favorite diagnoses and problems. Favorite categories can be created at the time you are saving a favorite diagnosis by selecting Create Category from the Favorite Problem List window.

  1. From the Favorite Problem List window, select Create Category.
  2. Enter the desired category name.
  3. Click OK.

Favorite categories can also be created from the main application Preferences menu.

  1. From the main application menu bar, select Preferences>Problem List….
  1. From the Problem List Option window, select Create Category.
  2. Enter the desired category name.
  3. Click OK.

Repeat steps 2-3 to create additional categories.

Removing Favorites

  1. From the main application menu bar, select Preferences>Problem List….
  2. From the Problem List Option window, select the desired category or diagnosis/problem.
  3. Select Remove.

Copying Favorites from another User

Problem List Favorites can be copied from another user. This feature is helpful if there are department standard diagnoses and problems that are recommended for use.

  1. From the main application menu bar, select Preferences>Problem List….
  2. From the Problem List Option window, select Copy From…
  3. From the Copy From window, select Other.
  4. Enter the last name of the user you wish to copy from.
  5. Select the user from the displayed list and click OK.

The other user’s favorites will appear on the right side of the Problem List Option window.

  1. From the other user’s favorite list, drag and drop the desired diagnosis/problem to “My Favorites” on the left side of the Problem List Option window. Desired diagnoses and problems must be moved over one item at a time. The Copy Selectedcommand may also be used to copy over desired items one at a time.

The Copy Allcommand will copy ALL diagnoses and problems from the other user’s favorites to your own list.
