User Card Application

(Use the same name as it appears in your passport.)

A.  Qualification: Register to become a NSRRC user on User Portal Website (http://portal.nsrrc.org.tw).

1.  Must be a member of NSRRC proposals: An applicant has to log in and out once at NEW User Portal http://tpsportal.nsrrc.org.tw/Default.aspx for database synchronization. P.I. could add the applicant to the member list in the proposal or Experimental Safety Approval Form.

2.  Have taken safety training and answered at least 70% of the questions correctly.

3.  Have uploaded a photo file in JPG or GIF.

B.  Two ways of taking safety training (approximately 50 minutes):

1.  Training online: Access the user website, download files from the website, and take the training. (Note: After you have passed the test, the system will ask you to enter your name/gender/ID number/e-mail.)

2.  Training at User Administration Office (Room T203, Ext. 8208): Please make arrangement for safety training by phone beforehand.

C.  Please upload a photo file in JPG or GIF for card issuing purpose. If users do not have a photo file, please take a picture at User Administration & Promotion Office.

D.  The user card should be picked up within 6 months. Please inform User Adiministration & Promotion Office three days before you pick up your user card. During office hours, please pick up at User Administration & Promotion Office (T203); during non-office hours, please inform User Administration & Promotion Office beforehand so arrangement of picking up the user card at the security guardroom located at the main gate can be made). If the user card is not picked up within 6 months, the user needs to retake the safety training. For verification purpose, please bring your passport when picking up the user card.

E.  If users cannot pick up the user card personally, please fill out “U25c User Card Pick up Authorization Form” and ask a person to pick up the card instead.

1.  The applicant has to sign on the Authorization Form and provide the R.O.C. ID/Passport.

2.  The pick up person has to show both of his/her and the applicant’s R.O.C. ID/Passport.

F.  The effective period of user cards is three years from the issue date. The user card can be renewed two months prior to its expiration date. The user should take NSRRC safety training to renew his/her user card.
